AAI Home Study Agreement
This agreement is made between the prospective adoptive parent(s) and Autumn Adoptions, Inc.
1. Autumn Adoptions Inc.(AAI) also known as the Agency agrees to conduct a home study investigation of the prospective adoptive parent(s). If AAI deems the prospective adoptive parent(s) to be unsuitable for adoption, the agency will inform them and provide no additional services under this agreement. The prospective adoptive parent(s) will be charged only a pro-rata fee for its services. The determination regarding the suitability is within the sole discretion of Autumn Adoptions Inc. The prospective adoptive parent(s) agree(s) to hold harmless Autumn Adoptions Inc. in the event that they are not approved for the adoption of a child.
2. The prospective adoptive parent(s) agree(s) to pay thehome study fee of$______. This amount is non-refundable in the event that the prospective adoptive parent(s) decide to discontinue the home study for any reason.Payment of the home study fee does not guarantee approval of a home study. Acceptable forms of payment are: personal or bank check or money order payable to Autumn Adoptions, Inc.
3.The home studyfee does not include the non-refundable application fee nor any fees for criminal history background checks, child protective service clearances, post placement services, or any other additional services or documents that may be required by your placement agency or adoption attorney.
4. This agencymay require a psychological assessment or any other documentation deemed pertinent before approving or disapproving an adoptive home. All required psychological assessments or documentation are to be provided at the expense of the prospective adoptive parent(s).
5. The prospective adoptive parent(s) agrees to provide any and all documentation requested by the Agency in order to assess their mental, emotional and financial stability and overall suitability in raising an adoptive child.
6. This agreement pertains to a home study investigation and in no way obligates the Agency to provide any other services such as an adoption placement. This agreement does not obligate the Agency to provide post-placement visits although we are happy to assist you should you require post supervisory services.
7. PLEASE BE ADVISED THAT HOME STUDIES ARE NOW VALID FOR 36 MONTHS AND MUST BE UPDATED EVERY 3 YEARS UNTIL SUCH TIME THAT A CHILD IS PLACED IN YOUR HOME.(International adoptions may require your home study to be updated sooner due to the expiration of your 171-H, also your home study should be updated if any significant changes occur to your home, financial or personal situation.)
8. Up to three copies of the home study will be provided at no additional charge.
9. Photos received by the agency may be used by the agency for promotional purposes (i.e.website, internet, seminars and brochures) but NO identifying information will be used under any circumstances without permission of the family.
10. Additional charges start at:
~ $150: Revision of home study for any reason after finalization.
~$25: Original signed/notarized copies of the home study report after finalization
~ $25: Any original document issued by Autumn Adoptions Inc. to meet inter-country adoption requirements.
We, the undersigned, agree with the terms of this agreement, and remit payment.
