Dear Parents and Students,

The end of the 2016-2017 school year is quickly approaching! The 8th grade teachers would like to ensure that the 8th grade students transition from middle to high school with some memorable moments. Below you will find a list which includes events, dates, and prices of planned 8th grade activities. Please indicate which items your child will purchase by marking the area beside that item and send in payments according to the payment schedule. Your child will submit all payments to his/her homeroom teacher. If you are paying by check please make your check payable to Dutchtown Middle School. Write the child’s name on the memo line and be sure a current phone number is on the check.

You will notice “money due” dates on the chart below. Should you decide to pay for items individually, please be sure money is turned in by those dates listed as we have purchasing to do and planning to do for each event ahead of time. You are welcome to write a check for all of the items you are purchasing at once. If money is not turned in by the due date, students will not be able to purchase the item. Also, due to the need to purchase items prior to the event, refunds will not be available.

Enclosed you will find a behavior contract that all students and parents will be required to sign in order to participate in the activities below. The purpose of the behavior contract is to ensure that all of the students understand that participation in the 8th grade activities is a privilege. We are confident that all of our 8th students will continue to exemplify the DMS tradition of excellence. If you have any questions please contact your child’s homeroom teacher.

If you would like to serve as a chaperone for any of the events listed, please indicate such on the backside of this page. Again, thank you for your continued support throughout this year.

Please note, the T shirt in the list below will be given to the students on the morning of May 12th for them to wear for their class picture. Since this is the same day as the dance we are in hopes that all will be here for the pictures. They will also be asked to wear these shirts again on May 25th for our 8th grade Dogwalk.

Please complete the 2nd sheet and return it to your child’s homeroom teacher with the money.

If I receive an office referral before any of the scheduled events, I will not be allowed to attend that event.
