In order to facilitate the payroll process of capturing FMLA (paid and unpaid leaves) we are contacting you for your assistance. An employee’s leave plans can change unexpectedly and without notification. Supervisors play a critical role by notifying payroll of these changes.

An accurate leave/time record provides CONTACT, HRIS (Payroll & Leave), with critical information that significantly impacts payroll.Notification enables the University to continue to pay employees when appropriate and avoid overpayments when paid leave has been exhausted.

It is our understanding that EMPLOYEE’S NAME’S leave is

1.scheduled to start on approximately DATE and end on approximately DATE.

OR

2.intermittent TERMS (i.e. 2hrs/wk) and will begin DATE through DATE or FISCAL YEAR.

If an employee has not already submitted leave records or if there are changes that affect leave and return dates, please keep HRIS informed, as any changes may have significant financial consequences for both the employee and the department.
