Curlabull Canine Training

I wish to attend the 6 week puppy and beginner dog training course run by Curlabull Canine Training at Burnopfield Community Centre - Thursday or Leadgate Community Centre -Friday. (delete as necessary)

I confirm that my dog is fully inoculated, regularly wormed and in good health. I understand that that should my dog be in contact with any infectious disease, I will not attend class until given the all clear by the vet. I understand that I am responsible for my dog at all times during the class and ensure my dog is under control and on lead at all times, unless directed otherwise by the trainer.
