CULTURAL, SCIENTIFIC AND ACADEMIC AGREEMENT BETWEEN THE FEDERAL UNIVERSITY OF MATO GROSO FOUNDATION AND THE UNIVERSITY...... AIMING TO SEND UFMT STUDENTS TO PERFORM INTERNSHIPS AND / OR STUDY PROGRAMS.

The Federal University of Mato Grosso, located in Avenida Fernando Corrêa, 2367, Cuiabá, Mato Grosso, Brazil, CNPJ. º33.004.540/0001-00, represented by the Rector, Professor Dr. Myrian Thereza de Moura Serra, RG. nº0310949-6, CPF. n°314.042.401-59, Brazil, appointed by Presidential Decree of 22/09/2016, hereinafter UFMT, and the University...... , located in (Address).:...... , City:...... , State:...... , Country:...... , represented by the (position/title and name):...... , Identity Document :...... , Nationality:...... , Act of legal representation:...... , hereinafter referred to as...... , sign this legal instrument in accordance with the legal provisions in force:

OBJECT

FIRST CLAUSE –The present Agreement sets out the form and conditions under which the agreeing parties propose to develop an academic, scientific and cultural cooperation program involving internships and/or a study program of UFMT’s students with the University …………………….., involving the areas …………………...

CONDITIONS

SECOND CLAUSE –In order to pursue the subject proposed the University ______and the Federal University of Mato Grosso Foundation undertake:

I - To designate those responsible for executing this Agreement.

II –To promote internship by giving the students access to the premises, facilities and activities of the specific department of the host institution, for the development of hands-on activities, and/or real situations of life and work related to the specific area.

III - To draw a joint work plan for internships and/or a study program for each student.

VI –To write an Activity Report, regarding the performed studies/internship carried out at the host institution, in order to validate the activities at UFMT, and to issue an academic transcript in the case of study programs.

INTERNSHIP AND/OR STUDY PROGRAM

THIRD CLAUSE –The internship and/or study program will be realized under the following conditions:

I –Sending of UFMT students:

1 - The number of students participating in exchange programs will be determined by previous notice;

2 –UFMT will select students to participate in the program. The host partner should approve such students;

3 - Students must have a good knowledge of the official language of the host university, unless otherwise stated.

4- The period of study should normally be one academic period or an academic year. Short internships, lasting up to three months, are acceptable, with previous approval and recognition of the student's course at UFMT.

5 –The internship period could, or could not, be carried out simultaneously with the study program, depending on the acceptance by the host institution.

6 –The students will bear all the expenses with transport, accommodation, food, health and life insurance, as well as academic fees, when applicable.

7 –The students may benefit from scholarships or reduction / exemption of academic fees at the discretion of the host institution, respecting the laws of the destination country.

FINANCIAL RESOURCES

FOURTH CLAUSE –UFMT will not cover costs of any kind to achieve the object of this agreement.

DURATION AND RECISION

FIFTH CLAUSE –The present Agreement will remain in force for five (5) years from the date of its signing; it can be declared invalid by any of the parties, by means of written notice at least sixty (60) days in advance.

PUBLICATION

SIXTH CLAUSE –The extract of the present Instrument will be published in the Diário Oficial da União within a period of 20 (twenty) days from the date of its signature. Such an initiative and expenses will be met by Federal University of Mato Grosso Foundation.

FORUM

SEVENTH CLAUSE –Any divergence that arises out of this Document shall be resolved by aarbitral council appointed by those who sign this Cooperation Agreement and by a third member chosen by the parties.

Thus agreed, the parties sign the present Agreement in 4 (four) copies of equal form and content, 2 (two) in the English language and 2 (two) in the Portuguese language in the presence of the below-mentioned witnesses who also place their signatures.
