CRIMINAL BACKGROUND CHECKS/ OIG/GSA
We at Aurora Health Care adhere to the guidelines established by the Wisconsin Caregiver Background Check Law in determining eligibility to participate in clinical rotations within Aurora clinical sites.
Careful attention is paid to the self-disclosure form the student/instructor will fill out requesting the Background Check. Please note if Question 1 is answered no (pertaining to past convictions or pending charges including ordinance violations) and a record comes back on that particular individual, we consider that a falsification of records, and therefore, will result in the person being barred from clinical rotations at Aurora Health Care.
Regarding Question #1:
Do you have criminal charges pending against you or were you ever convicted of a crime? :
• Please consider any interaction you have had with law enforcement such as the police or sheriff.
• For the purposes of completing the BID form, “convicted of a crime” includes felonies, misdemeanors, municipal ordinance violations and tribal court offenses even if the conviction is described as non-criminal.
• This also includes convictions resulting from a guilty plea or plea of no contest.
• If you received a ticket and paid a fine, excluding parking violations and most traffic tickets, it is likely that you received a conviction, and should note this on your form.
• If you have been informed that a conviction will not be on your record or has been expunged, you must still report it, as these almost always DO appear on your background check, and it will be considered falsification if you have not disclosed them.
• Most pending charges and most convictions, as described above, will not prevent you from participating in clinicals at Aurora. However, any failure to fully disclose them may be viewed as falsification of your record which will make you ineligible for participation in clinical experiences.
• If you are unsure whether or not you need to report an offense, it is always in your best interest to do so.
In addition to Criminal Background checks, we also require all students/instructors to be screened through OIG (Office of Inspector General) and SAM (Excluded Parties Listing System) checklists. The check consists of verifying that student/instructor names do not appear on these government agency lists. The appearance of a name on one of these lists would prohibit the person from participating in a clinical site at an Aurora facility.

Verification of students and instructors having completed the Criminal Background Checks as well as screening through the OIG and SAM with no results found is required prior to the start of clinical rotations. Any results found on the background checks or screenings need to be presented and approved by Aurora.
