COUNT 2 DIRECTIONS:

AU’S CIMC CONTACT PERSON:

  1. Please review the attached report for your Administrative Unit (AU) and make sure that every student who has been identified as Visually Impaired including Blindness in your AU who is receiving Vision Services as of January 5, 2015,is listed.
  2. Prepare Registrations for those that are not on your report, and Exit Forms for those who are no longer in your AU. Use the current forms found on our website at >click on Programs and Services >click on CIMC.
  3. Circulate the report to each TVI in your AU for their review and revision(s).
  4. After the TVI’s have made their revisions, forward to the Special Education Director of your AU for their final review and signature.
  5. Gather the report, including the cover page signed and dated by the Director, any and all Registrations, Exits and updated eye reports and return to Marla Wallace Raynor in the CIMC no later than January 30, 2015.

TVI:

  1. Review the report for your AU, noting any updates to the student data, directly on the report, taking note of any data field highlighted in gold. Don’t forget to update student grades for the 2014-15 school year and answer the State Assessment question for each student (If you missed that on Count 1)
  2. Even if there are no changes to the data, initial (legibly) where indicated for each student on your caseload.
  3. After each TVI in your AU has reviewed the report as indicated, return the report and any completed, signed Registrations, Exit Forms and current eye reports to your designated Contact Person, whose name is listed on the cover page, so they can return the entire Count 2 package to Marla Wallace Raynor in the CIMC by the January 30, 2015 deadline.

DIRECTOR of SPECIAL EDUCATION:

  1. Review the report for your AU, sign and date the report cover page, where indicated.
  2. Return report to the contact person listed on the cover page to return to CIMC by the January 30, 2015 deadline.
