Contractor Safe Work Permit Training Registration Form
The safety of your employees, our colleagues and our business is the key objective for requiring everyone to use Pfizer’s safe work permit process. In order to assist you with this process, please review this information prior to scheduling your employees for Safe Work Permit training.
The safe work permit specifies requirements for the different types of work your employees could be asked to perform. Below is a breakdown of required training and documentation by type of work.
ü General Work
a) Description: Any work not requiring the activities listed below.
b) Training Required:
(1) General Safe Work Permit.
c) Training Documentation Required: No documentation is required for this training; however any person completing work on-site needs to be informed of the general permit requirements.
ü Hot Work (Type I )
a) Description: (Torching, Grinding, Welding, Open Flame, X-Ray)
Note: Reference Fire Watch Attendant requirements.
b) Training Required:
(1) Safe Work Permit with Hot Work
(2) Hands-on Fire Extinguisher Training (Training must be from a qualified vendor within the last three years; contact Contractor Safety for vendor approval.)
c) Training Documentation Required: Certificate or class sign-in sheet of hands-on fire extinguisher training received within the last three years from a qualified vendor. Include this documentation with the Safe Work Permit Registration form.
ü Hot Work (Type II )
a) Description: (Non-Rated Electrical Equipment being used in an Electrically Classified Area)
Note: Pfizer has a chemical operation. Be advised due to this type of operation, a safe work permit is required for use of electrical or battery operated tools or equipment.
b) Training Required:
(1) Safe Work Permit with Hot Work
c) Training Documentation Required: No documentation is required for this training; however any person completing work on our site needs to be informed of the general permit requirements.
ü Confined Space Entry and/or Lock Out Tag Out (LOTO)
a) Description: (Energy Isolation, Line Entry, Duct Entry, etc.)
b) Training Required:
(1) Supplier provided training in Confined Space Entry and/or LOTO procedures within the last three years.
(2) Safe Work Permit with Confined Space and/or LOTO
c) Training Documentation Required: Certificate(s) or class sign-in sheet(s) of Supplier provided Confined Space and/or LOTO training received within the last three years. Include this documentation with the Safe Work Permit Registration form.
Refresher Training Requirements
ü Safe Work Permit Refresher Training is required annually.
ü Fire Watch Quiz Refresher Training is required annually for all contractors performing Fire Watch activities.
Fire Watch Attendant Training Requirements
ü Safe Work Permit with Hot Work
ü Hands-on Fire Extinguisher Training (Training must be from a qualified vendor within the last three years; contact Contractor Safety for vendor approval.)
ü Complete Pfizer Fire Watch Quiz annually.
