1. / General: Subject to the terms and conditions below, we are pleased to submit this Contract to furnish supervision, labor, equipment and materials (as noted) to perform the following pressure installations:

* Linestopping should provide a workable shutdown but does not guarantee that it will be 100%.
2. / Materials By Us: We will provide the following materials, delivered to your jobsite:A. / ______& ______/ Line Stop Fittings with completion plug and cap or blind
Quantity Size / flange.
Note: As Line Stop Fittings are of a custom design, once production has begun, you the undersigned, are responsible and liable for payment. Should the job be delayed thru no fault of Trumbull Industries, the fittings will be shipped and billed as material on site.
3. / EQUIPMENT By Us: We will provide the following equipment and machinery delivered to your jobsite:
A. / All Tapping and Line-Stopping equipment including temporary valves necessary to perform the Line-Stop.
4. / Labor and Supervision By Us:
A. / All technicians required to operate our equipment and machinery.
5. / Materials, Labor and Equipment By Others:
A. / All valves, pipe and fittings to be located downstream of our Line-Stop and tapping valves.
B. / A crane (“cherry picker”) or other suitable means of raising and setting our Line-stopping machinery, if needed.
C. / Assistance, as necessary, in transporting and/or rigging our equipment. It will be your responsibility to provide scaffolding/temporary work platform.
D. / Perform all excavation, backfill and site restoration, providing necessary barricades, sheeting, temporary shoring and bracing as required.
E. / For “Double” Line Stops, a buffer must be provided between the 2 stops. Please contact your Trumbull Sales Rep. for specific “how to” details.
F. / Maintain a reasonably dry, workable trench for underground installations.
G. / Arrange and pay for all permits, licenses, and inspection fees.
H. / Provide a suitable roadway from public thoroughfare to a point where the pressure tap is to be made. If conditions are such that our truck requires assistance in and out; you, or someone on your behalf, will provide that assistance.
I. / All environmental protection required.
J. / Make necessary trade union arrangements. Our technicians are unaffiliated.
K. / We are not responsible for damage to utilities after satisfactory completion of the above work.
L. / Provide means of pressure bleedoff (hydrants, branch connection, pressure Tap 4” min.) for draining and checking condition of Line-Stop.
M / An air compressor is required, 90 cfm (min.) or more; and with “Chicago” fitting connections/hoses.

O. / For A/C Pipe, the sleeve must be cradled in cement, after it passes our pressure test.
Any pipe size over 12” must be cradled in concrete after it passes our pressure test.

6. / DURATION AND DELIVERY: Our price is based upon expeditious completion of the work once we are required to report to the jobsite. We reserve the right to access reasonable charges to cover our equipment and standby expenses after an 8 hour day on the jobsite, due to delays beyond our control.
A. / Some Line-Stop Fittings are not in stock for immediate delivery and would have to be special ordered.
B. / We request 2 week’s notice for scheduling (ARO).
7. / DELAYS: In the event we are delayed on the jobsite for reasons beyond our control, we will assess a charge of $ ______per day to cover equipment and standby expenses. Any time over eight (8) hours will be billed at $50.00 per hour (for our 1 man crew).
A. / Included in delays is time spent in cleaning mains to remove any unknown debris or build up that does not allow the insertion of the Line-Stop plugging head.
8. / TERMS: Net Thirty (30) Days - No Retainage. 2% interest will be accessed on invoices over thirty (30) days.
9. / ACCEPTANCE: If our Contract is acceptable, please sign below as indicated and return one (1) signed copy to our office. No work will be performed by our personnel unless a signed copy of the Contract is in our possession. The above prices are based upon acceptance within thirty (30) days.
