CONTINUING PHYSICAL THERAPY EDUCATION SEMINARS

Kaiser Permanente Southern California, Orthopaedic Fellowship in Sports Rehabilitation is proud to offer a community continuing professional education seminar series for physical therapists in Southern California. This course series will include examination and treatment of the upper and lower quarter. All classes will begin at 8:00 a.m. each day. The cost of this11 day series is $900.00. Enrollment is limited to 10 physical therapists. Enrollment priority will be given to Clinical Faculty in APTA Credentialed Orthopaedic Physical Therapy Residency Programs and to individuals who have completed an APTA Credentialed Physical Therapy Residency.

*** Classes will begin at 8:00 am located at Kaiser Permanente West LA (6041 Cadillac Avenue, Los Angeles, 90034)

**** 71.5 contact hours or 7.15 CEU’s will be provided for this course series by Kaiser Permanente Regional Continuing Education Committee, an approved provider by the Physical Therapy Board of California.

2) Complete the registration form below and mail a check for $900.00 registration fee (check made out to Kaiser Permanente), Check must be post-dated May 2nd, 2015.

Full refunds are available 4 weeks prior to the first course. In the event of a cancellation less than 4 weeks in advance, no refunds will be given if we cannot fill your spot.
