Applications are invited for a 6 session post in Liaison Psychiatry based at RoyalAlexandraHospitalin Renfrewshire. Mental Health services in Renfrewshire are provided by integrated health and social work multidisciplinary CMHTs, supported by an intensive home treatment team and a primary care mental health team.

The post is based at RoyalAlexandraHospital and the Liaison team consists of 3 full time nursing staff and the Liaison Consultant. The liaison team is linked to the Renfrewshire Intensive Home Treatment Team who perform liaison duties at weekends. As part of an ongoing GG&C liaison review the post holder may have to share liaison cover arrangements with colleagues in Glasgow. On call responsibilities will be shared with other consultant colleagues on a current 1:50 rota.

The postholder will be supported by appropriate medical secretary time.

The successful candidate would be encouraged to take the opportunities available for further education and continuing professional development. There are active education and audit programmes within the service.

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Psychiatry or eligibility for specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training.

Consultant Liaison Psychiatrist

CURRENT LIAISON PSYCHIATRY SERVICES

Renfrewshire

The Renfrewshire Liaison Psychiatry service provides a self harm service and liaison assessments to all wards in the RoyalAlexandraHospital in Paisley including Maternity. It covers all ward referrals that require a psychiatric assessment with a same day service if referrals are received before10am.This allows the service to contact services out withRenfrewshire to access anyrelevant history on the patient.

They also work closely with medical and nursing staff on difficult cases including delirium and neuro psychiatric cases. They regularly participate in informal teaching with medical and nursing staff.They have a nurse ledoutpatient clinic on a Wednesday afternoon. This allows the service to offer shot term interventions and to assess a patient’s motivation to engage in longer term supports. The service is line managed by the Intensive Home Treatment Team who in addition to their function also provide Liaison cover at weekends and public holidays which ensures the RAH has a seven day week psychiatric liaison service.

Greater Glasgow

The Glasgow Liaison Psychiatry Service provides a service primarily to patients in General Hospitals throughout the Greater Glasgow part of Greater Glasgow i.e. the Western Infirmary, Gartnavel Royal Hospital and the Golden Jubilee Hospital in the West, Glasgow Royal Infirmary in the East, and the Southern General Hospital and the Victoria Infirmary in the South. It also provides services to the Vale of LevenHospital in Alexandria. It covers all in-patient ward referrals up to the age of 65, all self-harm presentations, urgent referrals on over 65 patients, and cover to the Emergency Departments during working hours. There are also out-patient clinics for patients with such things as Medically Unexplained Symptoms, and cases where psychiatric and physical morbidities are very closely linked.

Nursing Staff within GLPS also provide a service for assessment of in patient self harm cases at week-ends and Public Holidays

Managerially the Service is managed by a Senior Management Team consisting of the Lead Clinician, the Service Manager, two Clinical Nurse Managers, and the Team Administrator. It sits as a relatively autonomous service within the Glasgow City CHP. For local matters each consultant is also under the Clinical Management of their local Clinical Director

The principal hospitals covered by the three GLPS bases are:-

LIAISON PSYCHIATRY

Liaison psychiatrists specialise in the management of psychiatric problems in the general medical setting, such as the interface between physical and mental symptoms, psychological reactions to physical illness, medically unexplained symptoms (MUS), and the management of self harm in the general hospital.

There is greater variation in liaison mental health services across the country reflecting local demographics, needs and priorities, available resources and historic development of the services. This has resulted in a patchy delivery of liaison services with different models of delivery.

A liaison mental health service may include some or all of the following components:

  • Emergency Department liaison psychiatry
  • Self harm service
  • Psychiatric liaison service for general hospital adult (of all ages) in-patients
  • Out-patient clinics
  • Additional specialised services (e.g. drugs and alcohol, Chronic Fatigue Syndrome, MUS, psycho-oncology etc)

CLINICAL ROLES:

  • Understanding the interface between physical health and mental health (e.g. delirium, encephalitis).
  • Diagnosis and formulation of management plans in complex cases, advising medical teams on a appropriate integrated care.
  • Assessing and managing risk (e.g. suicide risk, violence/aggression, absconding etc) relating to psychiatric conditions in general hospital settings.
  • Bridging the gap between primary and secondary care with regard to the management of psychosomatic conditions in the community (e.g. medically unexplained symptoms).
  • Prescribing and giving advice to medical teams on psychotropic medication.
  • Providing expertise, and fulfil a statutory role, in managing medicolegal issues using Capacity and Mental Health legislation.
  • Understanding the medical issues in assessing patients with medically unexplained symptoms.

DUTIES OF THE POST

The Post holder will be responsible for providing clinical leadership primarily to Renfrewshire. The core service provided by Renfrewshire Liaison services is to in-patients and Emergency Departments, but we also provide limited out-patient specialist clinics. The Post holder will agree a Job Plan with the Lead Clinician of GLPS, and although the post is offered on a 5 Session Direct Clinical Care/I Session Supporting Professional Activity Contract, it is recognised that, due to the nature of the post, it is likely that this will require to be renegotiated

COVER ARRANGEMENTS

Cover for annual/study leave etc is normally arranged within the Service. The Liaison Consultant will have their clinical duties covered by Greater Glasgow AND Clyde Liaison consultants. Given the close relationship between the liaison and IHTT service in Renfrewshire, the liaison consultant would be expected to provide cover for the IHTT during periods of IHTT consultant absence.

Present Service Provision in Renfrewshire

Community mental health services in Renfrewshire are part of Renfrewshire CHP, and cover a total population of 181,481. The Mental Health Partnership for Greater Glasgow and Clyde supports and coordinates the delivery of adult mental health services delivered by CHCPs in four divisions across the Board area: South Clyde, South Glasgow, West Glasgow and Northeast Glasgow.

The Service General Manager for Mental Health in Renfrewshire is Katrina Phillips and the Clinical Director is Dr Alexander Thom. The Acting Community services manager is James Clocherty. The Associate Medical Director for Mental Health is Dr Michael Smith, and the CHP Director is David Leese.

Inpatient psychiatric services in Renfrewshire are provided at DykebarHospital, which has 37 acute inpatient beds at present. The hospital has access to a total of 8intensive psychiatric care unit beds based in Inverclyde. A total of 20 beds provide inpatient intensive rehabilitation and continuing care services for South Clyde.

Adult Mental Health Services are currently based round two joint health and social work community mental health teams (CMHTs) - a Paisley CMHT based at the Charleston Centre, Paisley and a West Renfrewshire/Renfrew CMHT based at Mile End Mill in Paisley.

The Paisley CMHT is based at the Charleston Centre, and currently comprises 1 Staff Grade, 1 Nurse Team Leader, 5.0 G Grades, 3.0 Staff Nurses, 2.0 Support Workers, 2.0 OT's, 1.0 Consultant Psychologist, 1.0 Social Worker Team Leader, 4.0 Social Workers, 1.0 Social work Assistant, and 3.0 SW Support Workers.

The West Renfrewshire/Renfrew Adult Community Mental Health Team currently consists of 2 job share nurse team leaders, 1 Social work Team Leader,5.0 G Grades, 3.0 Senior Nurses, 5.0 Support Workers, 1.0 Senior Occupational Therapist, 1.0 Grade A Psychologist, 4.0 Social Workers/MHOs and 1.0 Social Work Assistant.

The “Doing Well” team (12 clinicians including 0.2 WTE consultant psychiatrist) provides treatment for all patients presenting with a new episode of low mood in Renfrewshire.

An “Intensive Home Treatment Team” (IHTT) has been in operation since April 2008. The team provides emergency assessments, gate keeps all new admissions and provides home treatment where indicated. The team operates from 9am to 8pm weekdays and 10am to 5pm weekends and holidays. The team comprises of 1Consultant Psychiatrist, 1.8 Team Secretaries, 1 Team Leader, 9 Senior IHTT Practitioners (9 nurses) and 2 IHTT Practitioners (1 OT). 2 support workers.

There are separate teams for Liaison Psychiatry and Rehabilitation Services. Patients in Renfrewshire have access to specialist psychology, psychotherapy and addiction services. The service can access beds at the Peri-natal Unit at the Southern General Hospital, Glasgow. Forensic service provision includes a forensic CMHT for South Clyde. The Community Eating Disorder Service Is based in East Glasgow.

Child and Adolescent Services are based at JohnstoneHospital but the inpatient beds are provided at GartnavelRoyalHospital and YorkhillHospital respectively. There are 10 Learning Disability assessment/treatment beds and a Community Learning Disability Team.

ON CALL DUTIES

The post holder will undertake on-call duties, which are currently under review. At present Senior on call arrangements across GG&C are that there are two Higher Trainees and two Consultants on for GG&C. These are a Higher Trainee and a Consultant for North of the Clyde and the same for South of the Clyde. A degree of flexibility is, however, expected amongst the four. Current on-call is approximately 1 in 50

TEACHING AND TRAINING

Postgraduate

Although there is never a guarantee that trainees will be attached at all times,

Core Trainees and where appropriate, Higher Specialty Trainees are rotated through Liaison as part of their training, and the Post holder will be expected to provide clinical supervision, and to support the Educational Supervisor in arranging appropriate training. Where necessary, Clinical Training Supervision Training will be provided. There will also be opportunity for being involved in wider Postgraduate Training both with Psychiatric Trainees and Trainees in the General Hospitals. The Post holder will also be expected to participate in the Local Postgraduate Training at DykebarHospital.

Undergraduate

The postholder will be expected to provide clinical teaching and training for medical students on attachment to DykebarHospital. Liaison Psychiatry is particularly well placed to attract students to Psychiatry, and in recognition of this, the post holder will be encouraged to provide Taster Weeks to FY1/2 doctors, and to develop Student Chosen Modules and Electives for interested students

Nurses/PAMS

Although there is currently no formal expectation that the post holder will provide teaching and training to staff out with Mental Health, where opportunities to develop this are encountered they will be encouraged.

4.Teaching/Administration

The post holder will undertake the administrative duties associated with the care of patients and the proper function of the clinical team including appraisal, clinical and non-clinical supervision of medical staff and clinical supervision of non-medical staff. He or she will also be expected to participate in the broader aspect of administration and management within the Renfrewshire Division of Psychiatry and the Renfrewshire Community Health Partnership.

It is expected that the successful candidate will take part in Job Planning, Appraisal, Peer Review and Continuing Professional Development. Participation in audit or research is encouraged. The post holder will be actively involved in the teaching of undergraduate medical students. In addition the post holder will be expected to participate in the postgraduate teaching of medical staff and staff of other disciplines.

DEVELOPMENTS IN GG&C AFFECTING THE POST

It is envisaged that the new SouthGlasgowHospital will open in 2015, at which point both the Western Infirmary and the old Victoria Infirmary will move there, with the complete closure of the Western infirmary, including its Emergency Department, leaving only two Emergency Departments in the city, at Glasgow Royal Infirmary and the new SouthGlasgowHospital.

There is also to be a review of Liaison Psychiatry Services in general throughout GG&C. The review will cover services in Clyde (InverclydeRoyalHospital, the RoyalAlexandraHospital, and the Vale of Leven Hospital), and possibly the hours of operation of the Service. It is possible, therefore, that the post holder might at times be required to work in the Glasgow part of GG&C, or hours out with Mon-Fri 9.00-5.00
