Advanced Rank Contingency

If the rank or bid for tenure is not approved by the Medical School Promotion and Tenure Committee, additional options (including but not limited to appointment at a lower rank or being placed on a probationary period) will be discussed with your Department Head and the Associate Dean for Faculty Affairs.

Connection to University of Minnesota Physicians

This offer is made in connection with an offer of employment with University of Minnesota Physicians (“UMPhysicians”), the faculty practice plan affiliated with the Medical School. Although the Medical School and UMPhysicians are closely affiliated, they are separate entities. Therefore, the details of your offer and employment with UMPhysicians are set forth in a separate letter. The clinical practice of all physician faculty members of the Medical School is required to be performed through UMPhysicians – as a % appointee, this means that all of your clinical practice must be exclusively through UMPhysicians. Salary and Benefits

Your total initial annual compensation will be $. select one of the following options: (A: JUST BASE) This consists of two components: your first -year University base salary will be $__,000 and the remainder will be for your clinical practice duties and is paid by UMPhysicians as detailed in its separate offer letter.

You will receive a standard University faculty fringe benefit package which includes health, dental, life and long term disability insurance options and retirement, and optional deferred compensation and flexible spending accounts. Your basic employee medical, dental, and life insurance elections will become effective on the first day of the month following the first day of employment. Common Paymaster

The Medical School and UMPhysicians use a common paymaster. You will receive two paychecks but only one W-2. It is important to understand that you will have two employers at all times and that the terms of your employments, the policies applicable to your employments, as well as the compensation and benefits you receive, are separately determined byeach respective employer.

Assignment and Duties

Annual Review

Your performance will be reviewed each year via the annual faculty review process. Licensing, Credentialing, and Other Terms and Conditions

Your appointment is contingent upon meeting the policies and practices of the University of Minnesota and your eligibility for employment in the United States. If you hold an academic, research, or administrative position at another university or institution, your appointment at the University of Minnesota is subject to receipt by the University of your resignation letter to your current institution. The effective date of your resignation from your current university or institution must be prior to the first date of employment at the University of Minnesota. The receipt of this resignation letter is a precondition of appointment at the University of Minnesota.

In addition, this appointment and the effective start date are contingent upon starting your employment with UMPhysicians. Because clinical practice is an important part of your appointment, it is a fundamental expectation of your continued appointment that you maintain an unrestricted license to practice medicine in Minnesota, and employment by and full credentialing with UMPhysicians, University of Minnesota Medical Center and any other practice sites as appropriate.

Finally, in order to comply with University policy, this offer is contingent upon the successful completion of a background check.
