PROGRAM OVERVIEW
The 2016 State, Tribal and Territorial Agency Tobacco & Cancer Control Community of Practice (CoP) is a 7-month initiative comprised of 10 state, tribal and territorial agency teams (e.g., state, tribal, or territorial departments of behavioral health/mental health and substance use, tobacco control programs, cancer control/prevention programs, public health departments) that seek to learn and disseminate strategies to address tobacco use and cancer risk in people with mental illnesses and substance use disorders at the state, tribal, or territorial levels. Participating state, tribal and territorial agencies will gain access to national experts and educational materials to help them develop and implement evidence-based tobacco and cancer control and prevention practices among populations with mental illnesses and substance use disorders.
Participating CoP state, tribal, or territorial agency teams will:
- Identify strategies to better address tobacco and cancer control needs among populations with mental illnesses and substance use disorders;
- Incorporate specific state, tribal or territorial-level tobacco and cancer control strategies to address the needs of populations with mental illnesses and substance use disorders into Health Systems Transformation efforts, including but not limited to the following efforts:
- Preparing community behavioral health organizations to meet quality metrics and standards related to health systems transformation efforts such as Certified Community Behavioral Health Clinics (CCBHCs);
- Leveraging relationships with community behavioral health providers through state, tribal, territorial and/or local behavioral health provider associations;
- Supporting cross-systems collaborations among comprehensive cancer control programs, tobacco control programs and community behavioral health organizations (CBHOs).
Participating state, tribal, or territorial agency teams will participate in an in-person orientation and action plan development meeting at the Magnolia Hotel in Denver, Colorado, and webinars and/or group calls facilitated by national experts focused on topic areas identified as highest priorities by program participants. Throughout the course of the CoP, state, tribal, or territorial agency teams will have the opportunity to develop action plans for tracking and monitoring their tobacco and cancer control goals. Participating state, tribal, or territorial agency teams will be compensated $1,500.00 for their data collection efforts ($750.00 given upon acceptance; and $750.00 to be given after the team lead has submitted final report). NOTE: Compensation will be provided to the state, tribal, or territorial agency team lead for disbursement to the team.

The National Behavioral Health Network for Tobacco and Cancer Control (National Behavioral Health Network) project team greatly appreciates your commitment to achieving health equity among people with mental illnesses and addictions and is excited to support the fantastic work you do. Participation in thisCoPrequires significant time, dedication, thought, energy and enthusiasm to engage in this initiative. This ensures that we are able to support you in the best way possible. Therefore, we ask all CoP participating state, tribal, and territorial agency teams to agree to an important set of commitments as outlined below.
I hereby agree to:
- Assemble a CoP participating state, tribal, and territorial agency team consisting of decision makers who will provide adequate time, energy, and enthusiasm to participate in the process; complete the CoP team form; and return it to y Monday, May 9th, 8:00pm ET.
- Identify a project lead whowill be responsible for and/or ensuring that at least one team member is represented on all webinars, conference calls, and the in-person meetingThe project lead will be the main point of contact and responsible for submitting assignments (see below). Specific dates and times for all CoP activitiescan be found on page 5.
- Identify at least TWO (2)team members (and no more than FOUR (4)) that will participate in the in-person meeting inMagnolia Hotel in Denver, Coloradoon Tuesday, June 14th, 2016. One of the team members in attendance at the in-person meeting must be the project lead.
NOTE: All travel MUST be arranged through the National Behavioral Health Network’s Travel Agent. There will be no exceptions. The National Behavioral Health Network will only cover TWO (2) days and ONE (1) night of travel and lodging (June 13 – 14, 2016). Please follow the instructions for arranging travel and lodging on p. 11.
- Complete all pre-assignments and action plans (deadlines and instructions outlined on pages 7-10). The project lead will be the main individual responsible for submitting all pre-assignments and other deliverables by the due dates provided. Note: There may be additional pre-work requested to inform the content and delivery of the in-person meeting.
- Actively participate in sharing and dissemination of lessons learned, successes and challenges to CoP peersand to the broader National Behavioral Health Network membership.

Please indicate BELOWall team members who will be participating in the CoP in addition to who will be attending the in-person meeting. Each team should consist of at least one (1) representative from each of the following state, tribal, or territorial agencies: Behavioral Health/Mental Health & Substance Use Department; Tobacco Control Program; and/or Cancer Control/Prevention Program. State, tribal or territorial agency teams may also include representative(s) from other state, tribal, or territorial agencies including, but not limited to: state, tribal, or territorial public health department, chronic disease department, and/or Medicaid office. We look forward to seeing you in Denver, CO for the State, Tribal and Territorial Agency Tobacco & Cancer Control Community of Practice Kick-Off Meeting on Tuesday,June 14, 2016. This logistics sheet contains information that will be helpful to you as you prepare for your trip.
The meeting will be held on Tuesday, June 14, 2016 at the Magnolia Hotel. The meeting will begin at 8:00 am MST on Tuesday, June 14, 2016 and end at approximately 3:30 pm MST the same day.
Travel Instructions
If you will be flying into Denver, please use the following link through Avenue Travel. Should you have any difficulty, please feel free to contact Lynn Hendrick at or (703)548-5554 ext. 5646 and reference charge code “2390: State CoP” in your correspondence.
If you will be driving to the meeting, please note that the National Council will cover mileage at a rate of $0.575/per mile for travel in one’s personal car, not to exceed $650.00.
Please make your travel arrangements no later than Friday, May 15th. You must book your travel through Avenue Travel in order for us to cover these costs. Please notify Krystle Canare at if you decide to drive by car.

The National Council has a block of rooms reserved at the Magnolia Hotel. Check-in date is June 13, check-out date is June 14. Your room has been paid for, however, you must present a credit card when you check in to cover any other charges that you may incur beyond the cost of your room. . Once we receive confirmation of your travel plans from Avenue Travel, a room will be reserved for you at the Magnolia Hotel. If you choose to stay beyond these nights you will be responsible for the additional night(s) of lodging.

- From Denver International airport’s RTD train station, travelers can take the A Line - westbound in the direction of Union Station and exit at Union Station. From Union Station, travelers can take a complimentary car service offered by Magnolia Hotel. Travel Coverage Restrictions: Please read this section carefully.
Airline reservations should be booked as soon as possible, and no later than Friday, May 15th. Please be mindful that the National Council will cover tickets that are non-refundable, non-transferable, and the lowest available coach fare.
- Once the ticket has been issued, additional costs associated with flight schedule changes will be charged to the traveler.
- Travel is only paid from your origin and back to your origin. Any deviations from this route will be charged to the traveler.
- Only one checked bag will be covered by the National Council. Additional bags will be charged to the traveler.
- The National Council will cover up to $50 a day for meals. Please keep all receipts for your meal expenses.
- Travel reimbursement is for meeting related expenses only (i.e. for taxis to the airport from your origin, from the airport to the hotel and back, airport parking). Participants will only be reimbursed for a total maximum of $100.00. Taxi and airport parking receipts are required for reimbursement.
- Please note that rental cars require prior approval for reimbursement. Also, all reimbursement requests must be accompanied by receipts for all purchases over $5 and should be received by the National Council within 5 business days after the meeting.

Shelina D. Foderingham, MPH, MSW
Director, Practice Improvement
Shelina Foderingham is a public health social work professional who serves as a Director of Practice Improvement at the National Council for Behavioral Health where she manages the organization’s public health portfolio. She serves as the Program Directoron a number of projects including the NationalBehavioral Health Network for Tobacco & Cancer Control, the CDC OSTLTS Capacity Building Assistance for Public Health Professionals cooperative agreement. Ms. Foderingham also serves as faculty for a number of practice improvement projects focused on primary-behavioral health integration, health information technology (HIT) and clinical quality improvement. Prior to joining the National Council, she spent over seven years providing training and technical assistance to supporting health systems transformation efforts across the United States Gulf Coast. She holds a dual Masters in Social Work and Public Health from Tulane University and a Bachelor’s of Science degree in Psychology from Xavier University of Louisiana.

Margaret A. Jaco, MSSW
Senior Policy Associate
Margaret Jaco is a behavioral health professional who currently serves as Senior Policy Associate at the National Council for Behavioral Health. In this role, Margaret provides project coordination and other support to multiple practice improvement and leadership programs, including CDC-funded initiatives and the Reducing Adolescent Substance Abuse Initiative learning community, funded through the Conrad N. Hilton Foundation. Ms. Jaco provides project management and writing support for other federal and state grant proposals, and serves as the Project Manager for the CDC-funded National Behavioral Health Network for Tobacco & Cancer Control. Also at the National Council, Margaret previously served as Program Associate for Mental Health First Aid, the National Council’s premier mental health public education program. In this role, Ms. Jaco was responsible for providing tailored technical assistance, developing customized curriculum modules, and creating detailed reports. Prior to joining the National Council, Margaret served in various positions for behavioral health and advocacy organizations, including extensive work in the realms of affordable and public housing, Medicaid, and public policy analysis. During her graduate studies, Ms. Jaco authored two Federally-funded publications on topics related to homelessness.
