§ 1

1.  Subject of the agreement is the implementation of the students' practice program according to students’ specialties.

2.  Individual Civil Practice Program for student …………………………………………………….

will take place from……………..……..…to………………..…..… for up to 40 hours / week.

3.  Civil Practice Program, developed by the University is attached as Appendix 1 to this agreement.

§ 2

Company is committed to:

1.  ensure appropriate conditions to hold practices in accordance to the practice program;

2.  familiarize the student with the company working rules, regulations, health and safety policies and the protection and confidence policies in the company;

3.  permit supervisors on behalf of the University to control and monitor evaluation of the practice;

4.  prepare accident documentations, if a student has been injured during practical training in the company.

§ 3

The University is committed to:

1.  develop a schedule of students’ practice and familiarize students with the content;

2.  supervise and monitor evaluation of practice;

3.  insurance students against accident.

§ 4

Student is provided with clothing and shoes on his/her own and at his/her own expense. If
a student does not have the proper clothing, Company reserves the right not to allow student to begin the practice.

§ 5

All non-pecuniary matters arising in connection with this Agreement shall be settled amicably by the persons representing the parties.

§ 6

The agreement will expire on the date of completion of the practice, mentioned in § 1 section 2.

§ 7

In matters not covered by this agreement shall be governed by the Polish Civil Code and other applicable laws.

§ 8

Amendments to the Agreement must be in writing to be valid.

§ 9

The Agreement shall enter into force upon signature.
