Discrimination and Sexual Harassment Policy Statement

LA Business Personnel, Inc. will not tolerate any form of unlawful discrimination, including sexual harassment. Any employee who engages in unlawful discrimination or sexual harassment will be subject to appropriate discipline, up to and including termination. Complete & Accurate Information

I hereby certify that I have not knowingly withheld any information that might adversely affect my chances for employment and that the answers given by me are true and correct to the best of my knowledge. I further certify that I will provide true and accurate information to any of LA Business Personnel, Inc.'s clients to whom I am referred to. I understand that any omission or misstatement of material fact on this application or any other document completed for LA Business Personnel, Inc. or clients to whom I'm referred to secure employment will be grounds for rejection of this application or for immediate discharge if I am employed by LA Business Personnel, Inc. or any of their clients.

Temporary Assignments

A temp assignment can be as short as four hours and can last indefinitely. When you work as a temporary employee, you are an employee of LA Business Personnel, Inc. You are not to accept any kind of payment from our clients. You should not handle any negotiable instruments or cash without supervision. You are not to operate any of our clients' machinery equipment or motor vehicles. LA Business Personnel, Inc. is not responsible for any damage to your motor vehicle while working as a temp. When you work on the computer, do not change, add or omit any of the programs, formats, settings, fonts, etc. unless you are asked to.

Benefits

As a temporary/contingent employee, you are not qualified for any of the benefits that our client(s) offer to its regular employees, consultants or independent contractors. You are qualified for benefits LA Business Personnel, Inc. offers which include Medical Insurance with optional life, vision, and dental plans. Direct Hire

After you go on an interview through us, call your representative to discuss how the interview went. When you are offered a job, you must contact our office immediately. If you are offered a job within one year of your interview date or the last date of temporary assignment, you must notify us immediately.

Offer of Employment

LA Business Personnel, Inc. works hard to find a job for you. We need your cooperation and honesty. If our client(s) offers you a position (temporary/independent contractor and/or regular full-time), we must be notified immediately. If you are offered any type of employment within one year after the last date of assignment or interview, we need to be notified immediately. If our client(s) refuses to pay our invoices and/or placement fee, you must terminate your job when we notify you. If you decide to work for our client(s) without our knowledge/approval and/or after we have asked you to terminate your assignment, you may be liable for our fees and legal costs.

Attendance/Punctuation/Performance

It is important that you keep your appointment for an interview and make sure you are on time. We recommend arriving 10 min. earlier, if possible. Dress appropriately for your interview. Have a copy of your resume available. When you start a temp job, arrive to work on time (10 minutes early on your first day), dress appropriately, take initiative and do your best. If you don't have enough work, make sure you ask your supervisor for work. If you come across any problems or have any questions, call us. When you are on a temp assignment, if you are going to be late or out for a day, you must notify us. If you can't keep your interview appointment, call us in advance. If you report to a temp assignment or an interview and nobody is there or you are told that the assignment or interview has been canceled, call us from there. Do not go back home and call us from home.

Work Injury

If you are hired directly by our client (regular full-time/part-time), you are an employee of our client. Any work injury must be reported directly to client. If you work as a temp through us, you are an employee of LA Business Personnel, Inc. If you are injured at work, you must notify your immediate supervisor AND our office IMMEDIATELY. As a temp, you are covered under our worker's comp. insurance and must report your injury to our carrier.

At-Will Employment

I understand and agree that if I am employed by LA Business Personnel, Inc. or any of LA Business Personnel, Inc.'s clients, my employment will be at-will which means that LA Business Personnel, Inc. and their clients may terminate the employment relationship at any time, with or without cause and with or without notice. Likewise, LA Business Personnel, Inc. will respect my right to terminate my employment at any time, with or without cause and with or without notice.

Temporary Pay Rate

It is your responsibility to verify the pay rate for your temp assignment with our representative. Every assignment is different and may have a different pay rate. It is your responsibility to get your time-card signed by client and turn it into our office. Checks are available for pickup every Friday. If you to have your paycheck mailed to you, we are not responsible for any delays in mail delivery. You must wait for seven working days before we will issue a replacement check. If you lose your check, you will have to pay the appropriate fees for a replacement check.

Company Obligation

I understand and agree that LA Business Personnel, Inc.'s acceptance of this and future job applications does not mean that a position for which I am qualified is open or that LA Business Personnel, Inc. has agreed to hire me or send me out on interviews or temporary assignments. I understand that LA Business Personnel, Inc. is under no obligation to hire me or refer me to clients as the result of accepting this completed application. LA Business Personnel, Inc.'s clients are under no obligation to hire me either.
