Supported Living Coordinator

Nature of Work:

Under the supervision of Community Support Managers, the Supported Living Coordinator is responsible for providing supported living services to adults with developmental disabilities who live in their own home in the community. The Supported Living Coordinator works with other members of an individual’s support team to ensure high quality, person-centered support. All duties are performed in accordance with the Mission, philosophy, values and policies of Community Living Connections, Inc.

Job Responsibilities:

  • Work directly with individuals supported by CLC.
  • Communicate regularly with supervisor and other direct support staff regarding the needs of individuals supported.
  • Supervise, mentor, train and support Supported Living Staff, and other staff, who work in the home. This includes delegating tasks as appropriate.
  • Solicit and encourage the exchange of information between support staff.
  • Monitor work performance and appropriately address work performance issues with Supported Living Staff. Inform Community Support Managers of all ongoing issues or concerns with direct support staff.
  • Develop, and maintain, a high quality home environment which is conducive to the growth and development of each individual supported.
  • Assist in the development and review of case plans and annual summaries.
  • Assure the daily implementation of goals as outlined in an individual’s case plan.
  • Be available to individuals supported, and other staff, for guidance, problem-solving and support as needed.
  • Provide ongoing assistance, as needed, to individuals with their personal finances.
  • Assure the home is maintained in a clean, safe and healthy condition.
  • Provide and arrange transportation, as needed, for the individuals you support.
  • Work hours are expected to be flexible as determined by the needs of the individuals supported, necessary appointments, team meetings, trainings and emergency situations.
  • Complete and submit all required documentation as requested and required.
  • Be familiar with any state licensing regulations and procedures.
  • Provide general maintenance as needed. This may include lawn care, snow and ice removal and other upkeep. Notify Community Support Managers and landlords as necessary.
  • Maintain communication with Community Support Managers regarding any household or individual issues.
  • Schedule and lead monthly staff meetings. Communicate date and time to others as appropriate.
  • Submit a completed monthly staff schedule to Community Support Managersby the 15th of every month. Supported Living Coordinators are expected to help cover, and find coverage for, any open shifts.
  • Complete and submit monthly report to Community Support Managers as required.
  • Maintain all records and reports as they pertain to the people supported and ensure the confidentiality of these records.
  • Schedule medical, psychiatric, dental and other appointments as needed. Forward documentation of appointments to Community Support Managers .
  • Assist with and monitor food preparation, menu planning and grocery shopping. Guide individuals supported to make healthy choices based on dietary recommendations.
  • Assist individuals to create and maintain social relationships.
  • Maintain awareness of current community activities and assist individuals to utilize community resources.
  • Assist individuals to maintain contact with their family and friends.
  • Conduct fire drills and review evacuation procedures on a monthly basis.
  • Ensure appropriate personal protective equipment is in ample supply, in designated areas, and ensure proper use.
  • Ensure compliance with OSHA reporting procedures.
  • Attend and complete all required initial in-home, classroom and on-line training within the first 90 days of employment.
  • Attend staff and team trainings and meetings as scheduled or recommended by supervisory staff.

Skills, Knowledge and Abilities:

  • Ability to supervise staff.
  • Must be able to work independently with the ability to make sound decisions and exercise good judgment.
  • Must possess excellent oral and written communication skills.
  • Ability to establish and maintain effective working relationships with staff, individuals supported, families, general public and other parties.
  • Ability to represent CLC in a professional manner.

Physical Abilities:

The following physical abilities have been deemed necessary. As with all required abilities, any possible reasonable accommodation will be made should an employee be unable to perform a required duty based on a disability as defined in the Americans with Disabilities Act (ADA).

  • Ability to routinely lift 15 lbs. and on occasion up to 40 lbs.
  • Ability to stand and walk for extended periods of time.
  • Ability to bend, squat, and stoop.
  • Ability to physically perform all job related duties.

Training and Experience:

  • Must be at least 18 years of age.
  • High School Graduate, or equivalent, required.
  • A degree in Human Services, or related field, preferred.
  • Two years’ experience working with adults with developmental disabilities preferred.

Driving

  • Access to a vehicle in good working condition, a WI driver’s license, an acceptable driving record and proof of insurance, which meets CLC ‘s standards, is required for this position.
