Community Connector

You will be directly employed by Threshold and will work in partnership with five other Alliance partners to deliver the Targeted Prevention Service for vulnerable people within Stockport. This new service will offer holistic advice, signposting and support to vulnerable people with a wide range of needs, utilising an asset based approach where people areassessed in terms of their strengths or assets and supported to increase their independence and resilience. The service will also focus on increasing social capital and building community capacity and informal support mechanisms, thereby meeting a key aim of preventing, reducing or delaying the need to access formal and statutory services.

Members of the Alliance are Age UK Stockport, FLAG, Nacro, Relate GM South, Stockport Homes and Threshold.

OVERALL JOB PURPOSE:

The overall purpose of the role is to deliver and co–produce innovative strategies that enable people to fulfil their full potential and live independently in their communities. This will involve working collaboratively with wider partners and local people who are also developing or have in place existing social action and community solutions.

PRINCIPAL ACCOUNTABILITIES:

The following details reflect the content of the post at the date prepared. The post holder will be expected to adopt a flexible approach to the duties which may have to be varied, after discussion with the post holder, subject to the needs of the Alliance and in keeping with the general profile of the post. Consequently this job description may be revised from time to time.
The Community Connector will:
  • Work with individuals referred to the service in order to assist them to articulate their desired outcomes and provide a time limited service to enable them to achieve those outcomes within their own communities
  • Develop effective community links and networks and promote partnerships to create collaborative community-based solutions that will:
  • Increase an individual’s knowledge of the local assets and circles of informal support available in their communities
  • Develop empowerment through information, advice and local opportunities
  • Improve social action through the design and delivery of user-led initiative
  • Reduce social exclusion
  • Promote independence
  • Reduce social and emotional isolation and loneliness
  • Develop and disseminate knowledge of local groups, activities and networks that will inform and support the development of the Alliance digital self-service offer

  • To work on own initiative, managing and prioritising own workload under the direction of the PHP Manager
