Community Care Navigator – 2 FTE Posts

Pay : £20,528 per annum

Fixed Term Contract until 31st March 2017

Hours of Work:

35 hours per week (The service will work flexibly between 8.00am – 6.30pm, Monday to Friday)

JOB TITLE:Community Care Navigator

LOCATION:TBC – various GP surgeries within the Solihull Clinical Commissioning Group

RESPONSIBLE TO:Community Care Navigator Service Manager

RESPONSIBLE FOR:No line management responsibilities

JOB PURPOSE:To work closely with GPs and other health and social care professionals to support people aged 65 and over with mild to moderate frailty and their carers to access timely support and interventions and help them to navigate services that meet their individual needs.

To provide a holistic service from locations that suit peoples’ individual needs, which may include home visits.

To promote health and wellbeing, prevention and early intervention leading to improved outcomes for individuals enabling independence and reducing unnecessary use and dependence on statutory services.

To deliver a service co-produced with all partners and service users.

DUTIES:

  1. To actively seek out people who would benefit from care navigation, including those seldom heard, and those who may be socially isolated.
  1. To work closely with GPs, practice staff and other health and social care professionals to identify people who will benefit from care navigation.
  1. To accept and act upon referrals from a range of referrers.
  1. To ensure any safeguarding concerns are acted upon immediately.
  1. To facilitate holistic care planning and coordination of input from a range of professionals.
  1. To actively participate in Multi-Disciplinary Team Meetings.
  1. To identify and work with people’s assets, and explore individual and family situations through a holistic process, developing care plans based on individual needs and circumstances.
  1. To work with patients providing the tools and knowledge to enable them to find solutions in order to manage their own health, social care, lifestyle and wellbeing into the future.
  1. To be an active member of the practices within the group, building relationships and engaging with patient groups.
  1. To support people who have been discharged from hospital and ensure appropriate services are in place to reduce the risk of readmission.
  1. To develop a reference tool of local referral pathways, activities and services available in the local community, liaising with providers and keeping the information updated on a regular basis.
  1. To work as part of a team of Care Navigators and of Age UK Solihull and to collaborate and liaise with other services such as Dementia Navigators and the Community Advice Hubs.
  1. To ensure unpaid carers are identified and have access to services to support them in their caring role.
  1. Communicate and refer to partner agencies, working across the sectors and ensuring referral pathways are strong, relevant and appropriate, coordinating and monitoring referrals for those most in need and deemed most vulnerable.
  1. To support the development of systems required to implement the Care Navigator Service including identification of improvements in efficiency and service delivery as the project progresses.
  1. To maintain databases and comply with monitoring and reporting requirements.
  1. To ensure strict confidentiality at all times in accordance with the SCCG (NHS) Requirements.
  1. Ensure that gaps in service and unresolved operational difficulties are reported to the Project Manager.
  1. To attend any training or meetings deemed necessary.
  1. To undertake any other duties that may be required.

Additional Notes

All staff are expected to work flexibly in terms of location and role.

All staff have a particular responsibility for ensuring that Safeguarding Adults legislation and guidelines are adhered to, that the well-being of at risk individuals is promoted and that at risk individuals are aware of their right to protection from all forms of abuse.

Appointment to this post is subject to an acceptable DBS Check and references.

This post requires use of a car and a full driving licence.
