Notice of Request for Proposals for Professional Medical Services

Community Action Corporation of South Texas (CACOST) is requesting sealed bids for contract(s) for Professional Medical Services for the Breast Cervical Cancer Screening Program.

Bids will be received by Anita Rodriguez, BCCS Billing Specialist, no later than 5:00pm on May 22, 2015 at PO Drawer 1820, Alice, TX 78333, and shall be clearly marked "Professional Medical Services Proposal, Do Not Open."

1.  Scope of Work

CACOST will provide breast and cervical cancer services in the counties of Aransas, Jim Wells, Bee, Brooks, Kleberg, Duval, McMullen, Kenedy, Live Oak, Jim Hogg, Refugio, and San Patricio. The health center will need adequate lab services.

2.  Item Specification

Professional Medical Services will need to include the following: Radiology Exams

·  Screening mammogram (include direct digital image)

·  Diagnostic mammogram (unilateral, bilateral, include direct digital image)

·  Breast Ultrasound (unilateral , bilateral)

·  Needle Core Biopsy using imaging guidance

·  Preoperative placement of needle localization wire, breast, and additional lesion

·  Stereotactic localization guidance for breast biopsy or needle placement

·  Image-guided placement

·  Radiological examination, surgical specimen

·  Ultrasound guidance for needle biopsy, radiological supervision

·  Vacuum assisted device

·  Excision of breast lesion/preoperative placement of radiological marker (single and additional lesions)

·  Galactogram or mammary ductogram

Surgeons

·  Office consultation (30, 40, 60min)

·  Established follow-up visit ( 15 min)

·  Breast Biopsy (incisional, excisional)

·  Core needle biopsy

·  Needle aspiration

OB/GYN

·  Office consultation (30, 40, 60 min)

·  Established visit ( 15 min)

·  Colposcopy

·  Colposcopy with biopsy and endocervical curettage

·  Colposcopy with endocervical curettage

·  Conization of the cervix

·  Loop electrode excision

·  Needle Aspiration

·  Cryotherapy/cryocautery

Anesthesia

·  Anesthesia/excision of breast cyst, per point (up to 6 points)

Hospital

·  Outpatient facility fees for excisional breast biopsy
