Collaborator Information Section
Under the title add the question “Did a collaborator assist you with your attestation?”
The response options will be ‘Yes’ or ‘No’

If the provider selects no then no other questions will be presented and the provider will be able to move on with their attestation
The question “Did a collaborator assist you with your attestation?” is required. If it is not responded to then then display the error:
- “Response to the question is required”
If the question “Did a collaborator assist you with your attestation?” is Yes then display the additional questions and response boxes for
What is the name of your collaborator?
If known, list the phone number of your collaborator?

- What is the name of your collaborator?
- If known, list the phone number of your collaborator:
The question “What is the name of your collaborator?” is required. If it is not responded to then then display the error:
- “Response to the question is required”
The question “If known, list the phone number of your collaborator:” is not required.
The text box for response to the question “What is the name of your collaborator?” should be alphanumeric and allow for 100 byte entry
The text box for response to the question “If known, list the phone number of your collaborator:” should be numeric and allow for 10 byte entry
- If the provider enters anything other than 10 numbers display the error, “The phone number should be entered as 10 digits, 8888888888.”
Collaborator Information to the Workflow Application
Update the Show Details (Search Results) screen to display the Collaborator Information Section entered by the provider according to the payment year being viewed:

Update document type drop down box options on the Document upload Screens
Update the document upload screen to have the following text added as an option on the document type dropdown box:
- Collaborator Worksheet
