Job Title: Medical Records Clerk

Exempt: / Non-Exempt
Salary Information: / Commensurate with Experience
Supervision: / Director of Clinical Operations(DCO)
Work Location: / Point Reyes-Administration Building
Summary: / Under the direction of the DCO, performs the clerical tasks required to process medical records requests(incoming/outgoing). Will work with (Datafile or current contracted vendor)to ensure all records are attached properly into ECW patient files as well as other clerical functions.

Education and/or Experience:
·  High school education, prefer some college
·  Typing minimum 40wpm
·  Familiarity/experience in a medical setting preferred
·  Electronic medical records experience a plus
· 
Language skills:
·  Bilingual a plus
·  Communicate pleasantly and professionally;
·  Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to present information effectively in one-on-one and small group situations to patients and other employees of the organization.
Mathematical Skills:
·  Ability to add and subtract two digit numbers and to multiply and divide with 10’s and 100’s. Ability to perform these operations using units of American money and weight measurement, volume and distance. Ability to convert units of measurement.
Reasoning Ability:
·  Ability to apply commonsense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
·  Ability to submit solutions to problems/situations.
Certificates, Licenses, Registrations:
California Driver’s License
Other Skills and Abilities:
·  Organize and plan work efficiently;
·  Demonstrate good working relations with all members of the CHA organization.
·  Knowledge of medical terminology, grammar, spelling, punctuation
Scope of Work Responsibility: / ·  Responsible for processing all incoming and outgoing medical records requests in a timely manner and with accuracy, ensuring that records do not contain any other patient’s records other than the patient who signed the release.
·  Review, print and send all Kaiser encounters to Kaiser twice per week and highlight any immunizations given.
·  Reconcile labs
·  Check that documents routed by outside data vendor are assigned to the correct staff member. Route any faxed pharmacy requests or clarifications to nursing team.
·  Will act as CHA liaison to outside data vendor
·  May include other duties/projects as assigned.

While performing the duties of this job, the employee is frequently
required to sit for long periods; talk or listen. The employee is required to
use hands to sort, gather, assemble paperwork, folders, envelopes.

occasionally lift and/or move up to 10 pounds. Specific vision abilities
required by this job include close vision, the ability to adjust focus and
read a computer screen for long periods.
