NURSING CARE ASSISTANT/HEALTH UNIT COORDINATOR

  • Schedules return appointments upon patient discharge per protocol
  • Prepare and upload paper documents into the EMR
  • Collects patient data and records in electronic medical record (EMR)
  • Recognizes patient data that is not within unit clinical standards and/or new abnormal changes in patient condition and reports to supervising RN within specified time frame
  • Communicates patient information promptly, with 100% accuracy
  • Responds to patient /family requests & needs promptly (i.e. assistance to BR, turning, I&O)
  • Proficient in use of telephone, paging system, unit/ nurse call systems and PIN resources
  • Demonstrates Safety Awareness/Preparedness and Adheres to patient safety principles:
  • Collaborates with charge nurse to respond to emergency situations on the unit using downtime and manual backup system as needed
