Class 1: Who We Are and What We Do

Plymouth Community Healthcare General Practice service is provided under an Alternative Provider Medical Services contract with the NHS Commissioning Board. Our range of services are; long term condition management, smoking cessation, minor surgery, sexual health and family planning in addition to the usual GP and practice nurse services.

Currently the APMS services complete QOF reports on an annual basis, which are available in the public domain, either on NHS Choices or the Department of Health website. GP Patient surveys are also available on Department of Health website.

As part of Plymouth Community Healthcarewe are aware that the decisions we make effect the public and the spending of public monies, therefore we try to involve and consult with the public as much as possible.

General Practice currently produces adisclosure logof all received requests for information, under the Freedom of Information legislation. We do not produce any other lists or registers.
