Welcome to Claremont Clinic PMS Practice. This leaflet is a brief description of the Personal Medical Services we offer. Please ask at reception if you require any further information

The Aim Of The Practice

To provide comprehensive medical care for people of all ages living within the practice area and promote good health through self-help and prevention of disease. We hope to work with other community services to further this aim.

New Patient Registration

Claremont Clinic will register patients if the

practice is operating an open list. All patients

wanting to register with the practice must live within the designated practice boundary and be able to produce 2 proofs of address prior to registering.

Named GP

All patients at the practice will be allocated to have a named GP. Patients can book an appointment with any GP they wish to see unless they are away.

Newham CCG

Our services are provided to patients under a

contract with Newham Clinical Commissioning Group (Newham CCG). This governs the services we offer to our patients. The CCG can be contacted about any aspect of primary care at:

Non-NHS Services

Some services are not provided free on the

NHS; therefore, a small charge will be levied.

These include Medical Examinations, Insurance

Forms, Travel Vaccinations, and Private

Certificates etc. Our current price list for these services is displayed in reception.

We are a registered Yellow Fever Centre.

Training Practice for GPs and Medical Students

The practice was accreditated on 16 October 2013, as a training practice by the London Deanery. The GP trainers at the Clinic are Dr H Desilva, Dr Choudhury & Dr Begum.

Suggestions and Complaints

We are continually making efforts to improve our service and we welcome all comments . You can do this by leaving comments by either completing the slips at reception or accessing the NHS choices website.

For any complaints please address to the Assistant Manager.

Your Information

We store your personal health information on our computer system. This is strictly confidential to the team involved in your care and will only be shared with other parties if we have your written consent or are required by law to do so. You can access copies of your own records. Please ask at reception for information about this.

Disabled Patients

The consulting rooms, treatment rooms and WC’s (disabled toilet on the ground floor) in our premises are accessible to people in wheelchairs. The practice also has a lift in operation.

Patient’s Rights and

Responsibilities

You have the right to access the services we provide according to your clinical need.

You have the right to expect a standard of care that would reasonably be provided by one of the Doctors or Practice Nurses.

You have the responsibility to attend

appointments punctually and cancel if you are unable to attend. All staff and patients at the practice have the right to be treated with mutual respect. Any incidence of verbal or physical abuse or threats of violence will not be tolerated and the appropriate action will be taken by the Clinic.

The opening times of the practice are detailed below please note this is different to the consulting times as indicated:

Please use this time to access the practice for collection of prescriptions, make enquiries or book appointments etc.

Contacting The Practice

Patients can contact the surgery on the telephones from 9.00am – 1.00pm and 3.00 pm to 6.15 pm (Mon/Tues/Wed/Fri) Thursday 9.00am to 12.30pm.

Out Of Hours Calls

Weekend and nights are covered in addition to our normal working day. Please telephone 020 7511 8880 in an emergency. All calls on this number are recorded.

Home Visits

Please request a visit before 10.00am to allow the doctors to plan their rounds. Come to the surgery if it is at all possible. Urgent visits will be dealt with a promptly as possible.

These appointments can be pre- booked directly by the patients by calling the central booking office reception on 020 7511 2075.

Repeat Prescriptions

Repeat prescription may be obtained by written request at the surgery or by post. We do not accept telephone requests. We require 48 hours notice. Priority is given to housebound patients for whom we take prescriptions over the telephone.

The Practice provides an Electronic Prescription Service known as EPS. Prescriptions are issued at the discretion of the doctor, who may request that you attend the surgery for a review of your medication. Please use the right hand side of your prescription to order repeats.

Minor Ailment Scheme - Your local pharmacies are taking part in a scheme that offers an alternative way of etting treatment for less-serious illnesses, at the pharmacy (Chemists).

You need: Advice and where appropriate treatment from a qualified Healthcare Professional. And you need it without a long wait.

You want: Free medicines if you are exempt from prescription charges.

Appointments & Telephone Consultations

The practice will try to offer an appointment with a GP of your choice if available. However, it may not always be possible to make an appointment with your doctor of choice. If this happens, an explanation will be given. Doctors offer pre-booked and on the day telephone consultations please ensure that you provide the correct telephone number for the GP to make contact with you and ensure that you are available to receive the call. Each appointment is for ONE PERSON only.

Change Of Address/Personal Details

Please check with reception to make sure that we have a correct telephone contact number and address details for you. As mobile telephone numbers can change frequently however, please ensure that you keep the practice informed of any changes this has an impact if we are trying to contact you.
