I agree to perform the tasks outlined in the Dry Creek Conservancy Salmon and Steelhead Survey Procedure Fall 2010 to the best of my ability.
I certify that I have read and understood the General Guidelines and other procedures described in the Dry Creek Conservancy Salmon and Steelhead Survey Procedure Fall 2010. I further certify that I have voluntarily applied to participate in this volunteer activity and to perform those tasks with the knowledge that there is a risk that I may be injured in the course of performing those tasks, and I agree to assume that risk.
I further certify that I am fully capable of performing these tasks and that I am not aware of any physical condition or limitation which would preclude or impede the performance of those tasks.
I hereby agree that I, my heirs, guardians, legal representatives and/or assigns will not make a claim against, or file an action against, Dry Creek Conservancy or any of its agents, officers, or employees for injury or damage resulting from negligence, howsoever caused, by any agent, officer, or employee of the Dry Creek Conservancy as a result of my participation in this volunteer activity or service. In addition, I hereby release and discharge Dry Creek Conservancy, its agents, officers, and employees from all actions, claims, and demands that I, my heirs, guardians, legal representatives and/or assigns now have or may hereafter have for injury or damage resulting from my participation in these volunteer activities or services.
I have carefully read this agreement and fully understand its content. I am aware that this is a partial release of liability and an agreement between myself and the Dry Creek Conservancy, which I have signed of my own free will.
