CSHCN Service System Coordinator

Public Health Nurse/Contra Costa Public Health Nursing

Permanent, fulltime

Primary Duties

  • Liaison to Roundtable meetings, Early Childhood Leadership Alliance (ECLA), provider agencies and health plans
  • Track high acuity cases
  • Monitor system wide trends and assure CSHCN receive services in a timely and coordinated manner through interagency collaboration
  • Provider education re. referral tools, screening, accessing local resources
  • Promote, supportand participate in collaborative partnerships

Coordinator to:

  • Set goals and objectives of CSHCN service system care coordination
  • Attend Roundtable meetings, providing support and continuity
  • Receive referrals of high acuity cases
  • Monitor follow up activities between families, providers and key stakeholders based on identified needs, using partner databases
  • Maintain database of children with special health care needs and their families and the outcomes of treatment and care coordination
  • Convene care coordination meetings as needed for complex cases or identified trends
  • Communicate coordination and tracking activities to appropriate health plan
  • Educate providers about Contra Costa CSHCN service system and resources
  • Identify needed resources, problems, gaps in service continuum and make recommendations for improvement
  • Advocate locally for services for CSHCN, their families and providers
  • Develop strong working relationships with decision makers of agencies participating in Roundtable meetings
  • Promote high standard of care including family centered approach

Qualifications

Possession of valid license as Registered Nurse in the State of California, certificate as a Public Health Nurse in the State of California, and baccalaureate degree in Nursing from NLN accredited school.

Experience

Minimum three years clinical experience.

Knowledge, Skills and Abilities

  • Comprehensive knowledge and understanding of CSHCN service system in Contra Costa County
  • Leadership skills to coordinate various professionals and agencies to build consensus for better, more efficient system for CSHCN
  • Significant experience working with families of children with special health care needs
  • Excellent communication, facilitation and relationship building skills
  • Work effectively with County staff, advisory boards, community agencies, private physicians and general public
  • Demonstrated ability for assessment, evaluation and interpretation of medical information and care planning
  • Strong computer skills/ability to compile and an analyze data/organize material and write clear concise reports
