Dear Parents/Guardians:
Welcome to Chesterfield Elementary School. We will be providing care to your child for his/her health and safety issues while attending our school. As part of a comprehensive health program, the state of New Jersey requires a physical exam for students entering school. Attached you will find a physical form for you and your physician to complete to meet these requirements.
Since most insurance companies will only provide one physical within 365 days, your child’s last physical within that time period is acceptable.
As per the state of New Jersey, all children entering school must have had all the required immunizations. Please provide a copy of your child’s immunizations. Attached you will find a form indicating the necessary immunizations required before your child can attend school.
**YOUR CHILD MAY NOT START SCHOOL WITHOUT A COPY OF THEIR IMMUNIZATIONS OR A CURRENT PHYSICAL.
On the attached Health History Questionnaire, please note any limitations regarding physical activity. If your child has a cardiac condition, they will need medical clearance to participate in physical education. If your child has a food allergy or food restriction, please speak with the school nurse.
Our school physicianwill review the physical and health history forms as needed. If your child has a condition that will impact his/her health in the classroom, we will ask permission to share that information with the classroom teacher and specialists.
If your child requires medications at school, the medication forms must be completed by your family physician. These forms can be found on the school website on the health office web page under the link “School Medical Forms”.
