Main purpose of job:

Chest Heart & Stroke Scotland improves the quality of life for people in Scotland affected by chest, heart and stroke illness, through medical research, influencing public policy, advice and information and support in the community.


The Heart Failure/COPD Support Service was formed in 2004 and was recommended for national roll-out in the Scottish Government Better Heart Disease & Stroke Care Action Plan 2009. The Service provides a package of support for people diagnosed with heart failure and/or chronic obstructive pulmonary disease. Current project sites are Lothian, Lanarkshire, Western Isles, Highland and Glasgow.

The main role of the Heart Failure/COPD Support Service Local Co-ordinator is to directly co-ordinate service provision and manage a volunteer and client caseload across Glasgow; working in partnership with NHS Greater Glasgow & Clyde.

Most challenging part of the post:

§  Recruitment and retention of volunteers

Key responsibilities:

To lead the delivery of the Heart Failure/COPD Support Service within Glasgow, in collaboration with the Heart Failure Support Service (HFSS) Lead

To ensure links are developed and maintained with local public/voluntary sector organisations, Glasgow Heart Disease and Respiratory Managed Clinical Networks (and associated sub-groups) and volunteer centres, across Glasgow.

To lead the promotion of the Glasgow Heart Failure/COPD Support Service, in collaboration with the HFSS Lead

To contribute to the updating of relevant sections of the CHSS website, in collaboration with the HFSS Lead

To ensure the maintenance of up-to-date records across the Glasgow service and provide monitoring and evaluation/annual reports as required, in collaboration with the HFSS Lead

Volunteers

To share information with the CHSS Volunteer Development Manager via the HFSS Lead

To recruit, manage and develop a team of up to 30-35 volunteers within the Glasgow service

To undertake a matching process with volunteers and clients in a befriending relationship and monitor standards across the Glasgow service

To provide direct training and support to a defined team of volunteers and uphold our high standards of volunteer management and Investing in Volunteers standards, across the Glasgow service

To deliver training packages as devised by Befriending Networks and CHSS and contribute to the development and review of training across the services, in collaboration with the HFSS Lead

To coordinate and monitor the CHSS system of reimbursing volunteer expenses, across the Glasgow service

To provide both formal and informal support to volunteers as directed by CHSS policies and ensure that policies are adhered to across the Glasgow service

To ensure that monthly reporting and statistics information, with respect to volunteers, for all Heart Failure/COPD Support Services and associated services is collated and submitted on time for the appropriate internal reporting procedures

Clients

To co-ordinate referrals from healthcare staff and ensure appropriate matching and support and monitor the effectiveness of referral systems across the Glasgow service

To maintain confidentiality of clients’ personal and medical information and ensure that protection of sensitive data is maintained across the Glasgow service

To support the needs of clients and their families in providing the service within Glasgow

Forum meetings/Newsletter

To plan, organise and deliver patient/carer Forum Meetings in partnership with local NHS staff within Glasgow:

-  To secure speakers and presenters for Forum meetings

-  To co-ordinate venue bookings

-  To co-ordinate the Forum meeting on the day

-  To undertake a process of continual review and evaluation

To co-ordinate the production of the West of Scotland edition of the ‘Take Heart’ newsletter for patients/carers, produced four times a year in partnership with NHS Greater Glasgow & Clyde, NHS Lanarkshire and the HFSS team

To evaluate patient/carer experience of Forum Meetings/newsletter and compile reports as required by the HFSS Lead

CHSS

To work to agreed targets and personal objectives

To participate in departmental, regional, organisational meetings and associated groups, as required

To represent CHSS at Managed Clinical Network meetings and associated groups, as required by the HFSS Lead and/or Deputy Director/Director of Advice & Support

To contribute to the work of the CHSS Public Relations and Health Information Departments as required

To ensure the well being, health & safety of people visiting CHSS

To comply fully with CHSS Policies and Procedures

To participate in the CHSS appraisal and support and development process

To undertake training as and when required

To liaise regularly with the HFSS Lead and Deputy Director of Advice & Support

To undertake additional duties as directed by the HFSS Lead and the Deputy Director of Advice & Support


The holder of this position will have unsupervised one-to-one contact as part of their normal duties with adults at risk; therefore this position is subject to membership of the Protection of Vulnerable Groups Scheme.

Chest Heart & Stroke Scotland comprises several departments: Finance including Accounting and IT, Administration, Public Relations, Fundraising, Advice & Support and Community Stroke Services.

Summary of Terms and Conditions of Employment

Location: The HFSS Local Co-ordinator will be primarily home-based within the
Glasgow area. Occasional travel to CHSS Head Office (Edinburgh) will be required.

Term: Permanent

Salary: On a scale of £19,674 - £23,395 per annum

Grade 4, NJC Scale Points 20 - 26
New appointments are normally placed at the base of the salary
scale unless there are exceptional circumstances.

Hours of work: 32.5 hours per week (CHSS full-time). The working hours are Monday to Friday with some flexibility. Evening/weekend work will be required on occasion.

Flexible working under a system of managed time will be required.

Review of Work: The postholder is subject to the CHSS objective setting, supervision and appraisal process.

Annual Leave: 24-30 days per annum, dependent on length of service, plus 12 public and local holidays.

Travel: You will be required to use your own car on CHSS business which is reimbursed at a rate of 45p per mile. All other reasonable travel, subsistence and other necessary expenses are reimbursed.

Pension scheme: CHSS operates a contributory Group Personal Pension scheme, which all employees are eligible to join; contributions are on an age-related scale of 1% to 5%. There are also schemes which provide death-in-service benefit and permanent health insurance.
