CHECKLIST FOR REGIS SCHOLARSHIP APPLICANTS
Keep this page as a reminder for what you must do and when you must do it. Check off each item as you complete it.
_____Attend OPEN HOUSE - Wednesday, Oct. 7th (5:30-8:30pm) for all Bronx, Manhattan and Westchester Residents; Monday, Oct. 12th (1:00-4:00pm) for all other areas.
_____Give FORM 2-ACADEMIC PERFORMANCE RECORD to Principal or Guidance Counselor together with stamped envelope addressed to Regis Scholarship Committee.
_____Give FORM 3-SCHOOL RECOMMENDATION to one of this year's or last year's teachers together with stamped envelope addressed to Regis Scholarship Committee.
_____Fill out FORM 1-STUDENT APPLICATION FORM in ink and in your own handwriting. The form is due a minimum of two (2) weeks before the exam. Be sure to answer every question. Sign page 4. Have parent sign page 4.
_____Any and all further material for admissions must be submitted by mid-November.
_____Mail check or money order for fifty dollars ($50.00) and a copy of your BAPTISMAL CERTIFICATE with Form 1 to:

Include FORM 4-CONFIDENTIAL FINANCIAL STATEMENT, if your parents wish. Include stamped envelope addressed to yourself, if you wish acknowledgement. NO ADMISSION CARD WILL BE SENT.
_____Come to Regis for SCHOLARSHIP EXAM on Saturday, November 7, 2009. Bring two pencils and a pen. Enter school building at 60 East 85th Street, which is the closest entrance to the IRT Subway at 86th Street Lexington Avenue.
_____Come at 8:30 in the morning, if you live in Manhattan, Bronx, or Queens.
_____Come at 12:30 in the afternoon, if you live in any other area.
_____After exam, if your parents are waiting at Regis for you, meet them outside the school, in front of 55 East 84th Street, at 11:30 A.M. or 3:30 p.m.
_____Expect letter from Regis no later than Friday, December 4, 2009. If you do not receive a letter, phone Regis on Wednesday, December 9, 2009, between 9:30 a.m. and 3:30 p.m. PLEASE DO NOT PHONE BEFORE THIS.
_____Except for the information given in the letters sent to applicants giving the decisions of the Regis Scholarship Committee, no other information is available to applicants, parents, schools, or anyone else. The deliberations of the Regis Scholarship Committee, and all information it receives from the applications, the exams, and any other materials submitted are completely confidential and are communicated to no one. PLEASE DO NOT REQUEST ANY OF THIS INFORMATION.
_____Keep SATURDAY, JANUARY 2, 2010 free for INTERVIEWS in the event that you are selected as a Semi-Finalist
