Complete this checklist and bring it with each of your packets to the Office of Faculty Affairs, G.420 MSB, by September 1. Approved promotions/tenure will become effective one year later on September 1.

____1. Lengthy letter from department chair addressed to the Dean to include the following areas: (all)

___rank, track, pathway, department, division** ___evidence of department review**

___ education and academic background ___duties, qualifications ___clinical activities ___teaching

___research ___academic/scholarly activities ___university/community service ___peer esteem

(**Please make sure the correct rank, track, pathway, department,and division of Internal Medicine, Pediatrics, and Surgery,are included in the lst paragraph of the department chair’s letter, in addition to evidence of department

review, which mayalso be in a separate letter).

____2. Three (3) internal (UT-Houston) reference list and letters, outside the candidate’s departmentat the same rank or

higher than that proposed for the candidate (no Assistant Professors).

(These letters should be addressed to the department chair and included with the packet when it is reviewed by the

department review committee. If your department does not have a department review committee, please form one.)

____3. External References--LIST only (not reference letters) of 6 external references at the same rank or higher than

that proposed for the candidate (no Assistant Professors), with all information completed on the attached (new!) reference list form.

____Confirm that all reference information has been verified viainstitutional websites.

For tenure track candidates, 3 of the 6 external references should not have worked or trained in the same department as the

candidate.

____4. CV in the UT-Houston format (no “submitted” or “in preparation” papers)

____5. Copy of completed Criminal Background Check (CBC) authorization form. Submit CBC form to HR and save

CBC # and approval date for 9/1 panels.
