The Chapter Achievement Awards were created to highlight the achievements of chapters in the areas of Education, Organization, Communication and Community.

Beforegetting started, please read carefully through the instructions below, even if you have submitted an application before, as the Achievement Application questions have changed.

How to Apply:

  • Make sure to select oneawardcategory from the drop down menu.
  • You can change the award category at any pointup until the submission deadline on January 9, 2013 at 5pm ET.
  • Before submitting your application, please review your supporting documentation to make sure it is relevant to the application.
  • All answers must be completed on the online submission form.
  • To prevent losing your answers due to technical/computer problems, it is recommended that you prepare and save a draft of your submission in a word processing document before entering the information online.
  • When you have completed your preparations, gathered your supporting documents and are ready to submit your application:
  • Copy and paste your question responses onto the online submission form; and
  • Upload your supporting documentation before transmitting your application.
  • Once your application has been submitted, you will no longer be able to make any changes to the supporting documents.
  • Please note that Incomplete applications will not be accepted.

Submission Requirements:

  • All entries must reflect activities from January 1 through December 31, 2012. Activities prior or subsequent to these dates should NOT be included, and will not be considered.
  • A Chapter cannot apply withinthe samecategory for which they received an award for in the year prior.
  • Submissions should represent activities that are portable from chapter to chapter. The application should highlight best practices that can be shared and may be duplicated among other chapters.
  • By completing an award application, you agree to the submission becoming the property of RIMS, and that RIMS may post and/or publicize it in any media it determines, in its discretion, to be appropriate.
  • Multiple award winners are anticipated and encouraged! Chapters are not competing against other chapters. Awards will be given based on individual chapter achievement - measured against a specific award point threshold.

Supporting Documentation:

  • Supporting documentation must be submittedasone (1) .pdf file, no larger than 5MB.As data file size is limited, please choose your supporting documentation carefully to achieve the clearest understanding of your application using the fewest and most relevant attachments. Tip: Color documents tend to be larger in size then black & white documents.Please provide legible black and white documents whenever possible.
  • Supporting documentation should be well organized, titled and numbered to reflect the question(s) it supports.

Submission Confirmation:

  • RIMS will send an email by January 9, 2013 confirming your submission and supporting documentation have been received. You may also contact RIMS Chapter Services to confirm it has been received.

Notification:

  • Notification of winners will be made to the Chapter's Board of Directors in care of the Chapter President.

Judging:

  • Your submission must receive a minimum cumulative score of 75 to be considered for the Chapter Achievement Award.

Below are the updated application questions. Please review them carefully before beginning to prepare your application.

  1. Provide a brief explanation of the achievement for which your chapter has applied. (5 points)
  2. Why does the Chapter believe this achievement is worthy of an award. (5 points)
  3. Describe the process used to establish the goals.(5 points)
  4. Describe each of the key processes, activities, and resources used to achieve these goals (20 points)
  5. State the goals that were set for this achievement. (10 points)
  6. How were these goals different from those in the past? (5 points)
  7. How was achievement measured for each goal? (10 points)
  8. Describe how the Chapter performed with regard to each of its goals (did the chapter exceed, meet or fall short of the goals). (20 points)
  9. Describe what the Chapter learned from this undertaking, and how this will impact future endeavors. (10 points)
  10. What part(s) of this achievement do you consider novel, unique, or innovative and, therefore, deserves consideration as a best practice that other chapters might benefit from? (10 points)
