Target Audience:

Target Audience

This training is for all interested individuals and specifically those who provide care to older adults, such as nurses, social workers, case managers, nursing home administrators, personal care home administrators, aging and mental health workers.

Educational Objectives:

At the completion of this seminar, participants will be able to:

·  Discuss the interconnections between chronic diseases such as diabetes, cardiac disease and arthritis, and mental health.

·  Summarize the outcome of treatment of mental health issues and its effect on these chronic diseases.

·  Critique evidenced-based practices that integrate mental and physical health care.

·  Identify barriers related to physical health and mental health in regards to diverse populations.

·  Review prevention strategies and lifestyle practices for healthy aging.

Continuing Education Credits Applied For:

Registered Nurses:

Through our partnership with P4A, this event is eligible for 4.0 hours of Nursing continuing education credits.

Licensed Social Workers:

This program has been approved by the Shippensburg University of Pennsylvania for 4.0 Social Work continuing education contact hours.

Case Managers:

This program has been pre-approved by the Commission for Case Manager Certification to provide continuing education credit to Certified Case Managers (CCMs).

Nursing Home Administrators:

The State Board of Examiners of Nursing Home Administrators has approved this program for 4.0 continuing education hours. Approval number is NHCE101672

Personal Care Home Administrators:

Application has been approved by the Pennsylvania State Department of Public Welfare for Personal Care Home Administrators for 4.0 hours.

Requirements for credit:

Full attendance is required at the workshop to receive CE credits or hours for above professionals. Partial credit will NOT be awarded. Late arrivals or early departures will preclude awarding CE credits or hours.

·  Attend/participate in the educational activity and review all course materials.

·  Complete the CE/Evaluation form.

·  Return the completed form to your conference host at the conclusion of the event.

·  Participants will be mailed a statement of credit within 4 weeks.

Content for this presentation was prepared by:

Ms. Linda Shumaker, RN-BC, MA, is a registered nurse currently serving as Outreach Coordinator of the Pennsylvania Behavioral Health and Aging Coalition. She has extensive clinical experience working in community mental health, geriatric assessment teams and long-term care. In addition, she has been an instructor of psychiatry at the Penn State College of Medicine. She has served as a founding member and former Executive Director of the Pennsylvania Behavioral Health and Aging Coalition, as a member of the Older Adult Committee of Pennsylvania Office of Mental Health and Substance Abuse Planning Council, and as a member of the Long Term Care Subcommittee of the Medical Assistance Advisory Committee. She has also previously served on the Board of Directors for the Pennsylvania State Alzheimer’s Association and the Advisory Board for the Central Pennsylvania Chapter of the Alzheimer’s Association.
