FUREVER AFTER RESCUE

ADOPTION APPLICATION

*Please note: You must be 21 or older to adopt from us.

FurEverAfter Rescue appreciates you taking the time to accurately fill out this application. Please realize that we ask you for this information to assist us in matching owners with dogs, thus making the correct fit! We will review your application. Veterinary and personal references will then be checked. Once we have completed that step, we would ask that you allow us into your home for a visit and bring the dog with us that you are inquiring about. This application, the personal and veterinary references and your home visit are all factors in the approval of your application. If you have another pet, the home visit would be the time that we would do a meet & greet to introduce your animals to the new dog. The home visit allows you the chance to see him/her in the setting they will be living in if the adoption goes through, and lets you take the time to decide if you are ready to make alifetime commitment to your new pet.
