DENTAL INSURANCE PLAN
- Two Plan Options Available
- Freedom Basic Plan – provides coverage for some of the more common dental procedures
- Freedom Advanced Plan - provides valuable protection and provides increased benefits over the Basic plan
- DHA Provider Network – with freedom to choose any dentist including specialists
- $1000 max per person, per benefit year
- $50 deductible per person, per benefit year
- Individual Monthly Premium Cost for Basic plan is $13.07
- Individual Monthly Premium Cost for Advanced plan is $21.44
- VSP-Vision Discount Services comes with either plan – no additional charge

HOLIDAYS
- The Center for Human Services honors 11 paid holidays per year.
- Holidays are: New Year’s Day, Martin Luther King Jr. Day, President’s Day, Good Friday, Memorial Day, Forth of July, Labor Day, Thanksgiving and the day after Thanksgiving, Christmas Eve and Christmas Day.
- Full Time Employees receive 8 hours, Part Time III Employees receive 6 hours, Part Time II Employees receive 4 hours
PENSION PLAN – 403(b) Annuity Plan
- To be eligible you must work 90-days and work at least 1000 hours in the plan year (June-July)
- Agency contributes a % of gross wages based on the following schedule: (partial listing)

Sick leave time accrued monthly at the rate of 6 hours per month, with a max of 180 hours balance
Part Time II Employees - 20 - 29 hours per payweek
Sick leave time accrued monthly at the rate of 4 hours per month, with a max of 120 hours balance
PERSONAL LEAVE DAYS
Full Time Employees - 40 hours per payweekYou are eligible for 1 personal leave day in a calendar year as long as you have 96 hours of sick time accrued. You may request a second day if accrued sick leave balance is at least 144 hours.
Part Time III Employees - 30 - 39 hours per payweek
You are eligible for 1 personal leave day in a calendar year as long as you have 72 hours of sick time accrued. You may request a second day if accrued sick leave balance is at least 108 hours.
Part Time II Employees - 20 - 29 hours per payweek
You are eligible for 1 personal leave day in a calendar year as long as you have 48 hours of sick time accrued. You may request a second day if accrued sick leave balance is at least 72 hours.
WORKERS COMPENSATION INSURANCE
- Employees are provided with a comprehensive workers’ compensation insurance program. Employees who sustain injuries or illnesses on-the-job must inform their supervisor immediately to qualify for coverage as quickly as possible.
