**It is very important that you keep a copy of these designated assignments.

Employers must e-mail, fax, mail or send the evaluation by the student in a sealed envelope. Students should ask the employers if it has been returned. Students are responsible for delivering the evaluation in enough time for it to be returned by the appropriate due dates.

Work-Based Learning Portfolio

A Portfolio is required for all students enrolled in the work-based learning program. The Portfolio notebooks will be kept on file in the Work-based learning office. The students will have access to their portfolio.

**The Portfolio will be your final for the work-based learning program. **

The Portfolio will consist of the following items:

  1. Cover Page
  2. Letter of Application
  3. Resume
  4. Thank you letter
  5. Copies of Work-Based Learning Application
  6. Pictures of you at work site
  7. Copy of a Post-Secondary Application completed
  8. Copies of all (4) four Employer Evaluations

See Mrs. Ausley for examples of work due.

Grading Scale:

GRADING SCALE
PROJECTS (Portfolio and Journal Assignments) / 30%
EMPLOYER EVALUATIONS (Technical Skills and Employability Skills) / 40%
WORK/WAGE REPORTS / 20%
WORK ETHICS (attending one WBL meeting per month and signing in or out daily) / 10%

**Please refer to the class syllabus for when each assignment will be due. The assignment will be accepted two days after due date without penalty. If it is submitted after the 2 day extended time, (3) points will be deducted for each day it is late. **

**Failure to successfully complete ALL ASSIGNMENTS may jeopardize future participation in the Work-Based Learning Program.

**IT IS YOUR RESPONSIBILITY TO CALL YOUR WORK-BASED LEARNING COORDINATOR WHEN YOU WILL BE ABSENT FROM SCHOOL. Each week of school is worth 100 points (20 points for each day). Failure to call your Work-Based Learning Coordinator or your employer will result in a deduction of 20 points from your weekly attendance grade. Failure to sign-in or sign-out with your Work-Based Learning Coordinator will result in a deduction of 20 points each day from your weekly attendance grade. In addition, you will also be marked absent.
