APPLICATION COMPLETENESS

This form was developed to make sure your packet is complete before it is mailed. Please go through your packet prior to be mailing and check this list off. If all the blanks are checked, your application is complete and ready. All packets are due by end of business on Friday (5:00pm) – April 7, 2017.

Application Form (Exhibit A):

Budget (Exhibit B):

Did you complete the budget form? Sign and dated?

Project Selection Criteria, Review Committee Contact (Exhibit D):

Did you justify your project in this section?

Did you indicate if you wish to present your project to the Review Committee Information?

Did you include match information for your project on this form?

Maps (Exhibit E):

Did you include a map of the areas and trails in your system?

(Exhibit F): Project Illustration

(Exhibit G): Letters of Support

Exhibit H): Additional Terms and Conditions – requires signature

Did you include photos, drawings, engineering plans and/or diagrams of the project? You can also include any vendor quotes in this section.

Sending the Application to Colorado Parks and Wildlife:

Mail 1 complete, signed copy to Colorado Parks and Wildlife – must be received by end of business (5:00pm) on Friday April 7, 2017.

Please note: We are not including the Environmental/Land Manager Approval forms (Exhibit C), as these are not required for equipment purchases. Please contact us if you feel you will still need these forms. Otherwise you can expect to see them in your Grooming Application package.
