Notes re: information required for the Learner Registration Form

1.  Each learner should have been allocated a personalised ULN number* when s/he registered for the course. This number should be listed here. (Note: in Scotland, these are called SCNs)

2.  Please ensure that the surname is spelt correctly (see 3 below)

3.  Please ensure that forenames are spelt correctly. There will be an administration charge for the issue of new, amended certificates.

4.  The date of birth is a mandatory requirement for awarding bodies and must be completed.

5.  Information should be entered in columns with the number (5) as follows:

Gender: M=Male; F=Female

Ethnic Minority: 10=White, UK heritage; 11=White, European; 12=White, known; 19=White, type unknown; 20= Black, Caribbean heritage;

21=Black, African heritage; 22=Black, other; 30= Indian; 40=Pakistani; 50=Bangladeshi; 60=Chinese; 90=Other, known; 98=Preferred not to say;

99=Ethnic information not sought.

Special Needs: 1=Candidate considers him/herself to have special requirements; 2=Candidate does not consider him/herself to have special requirements;

3=No information provided by candidate.

6.  Qualification Code: Use the Codes for the appropriate qualification that is being taken, i.e. IBSL1; IBSL3

7.  Unit Code: Use the Codes for the unit that is being taken, i.e. IBSL1A3; IBSL3ADC

8.  Please send this form to IBSL, 30 Museum Street, Warrington WA1 1PG with the appropriate total fee (if not already paid).
