To properly assess the success of the Seminar and to provide valuable feedback for future events it is recommended that you provide all attendees with an Evaluation Form. Below is a suggested series of questions that could easily be expanded on or tweaked to best serve your needs. Please keep in mind that most people won’t want to take too much time to answer too many questions. In question number 2 answer j please insert the particulars related to your event’s panel.
