JOB TITLE: Information and Advice project worker (Care Act)

ACCOUNTABLE TO:Head of Campaigns and Community Business Unit

RESPONSIBLE FOR:Delivery high quality information and advice service

LIMITS OF AUTHORITY:TO WORK WITHIN THE AGREED FRAMEWORKS OF HARROW MENCAP INCLUDING POLICIES AND PROCEDURES, BUDGETS AND RELEVANT LEGISLATION

MAIN TASK

/

DESCRIPTION

To provide Care Act information and Advice /
  • To obtain and process service users referrals received though the different referralchannels from clients, delivery partners and statutory services
  • To provide information, advice and support regarding matters relating to the Care Act 2014 and wellbeing
  • To enable service users to make informed choices about decisions regarding themby providing them with accurate and accessible information.
  • To provide individual casework for those who need it.
  • To distribute service evaluation forms to clients who have received a service, in line with partnership arrangements.
  • Signpost and support service users to appropriate services
  • Liaise with statutory services to enable service users to access services

Develop and maintain data systems /
  • To develop and maintain systems that accurately record inputs, outputs and outcomes that meet the needs of both the project , and commissioners
  • Provide timely management reports and information

  • To be self sufficient in own administration including the production of reports, upkeep of records and production of accessible information using relevant IT.

  • Adhering to Harrow Mencap’s financial systems and procedures.

Training
/
  • To participate in training offered by Harrow Mencap using acquired skills to enhance own work performance.
  • Undertake any formal qualifications necessary for the role
