What approach has been taken?
An adaptable (and still adapting) one.
Started with NHS CFH defined, Spine-based CAF Messaging, currently in test with locally defined messaging, hopefully implementing ITK-based CAF Messaging (still nationally defined, but implemented and assured locally).
How information sharing happens – high-level business processes
Initially scope was confined to Adult Social Care and Mental Health exchanging information. CPA Assessment data, recorded by Camden and Islington Foundation Trust Mental Health teams (using RiO, under BT control) needs to be transferred to London Borough of Camden Adult Social Care teams (using Corelogic’s frameworki), both for reporting purposes (e.g. RAP) and for coordinated care (e.g., whilst assessments are recorded in RiO, commissioning of services is done in frameworki).
Currently, duplicate (if not triplicate) entry of data across various systems.
Extending scope of project to include GP referrals to Adult Social Care, and Acute hospitals sending Section 2 and Section 5 messages to Adult Social Care.
What the anticipated benefits are
More timely and more accurate recording of Mental Health data in Social Care. Better holistic picture of the patient/service user.
More timely, more accurate and appropriately detailed GP referrals to Social Care.
More timely Section 2/5 Delayed Discharge notices to Social Care, enabling more timely responses.
What challenges have been encountered?
Although Corelogic built and demonstrated CAF Messaging to send and receive CAF Assessment and Care Planning messages to and from other systems, including the Summary Care Record, we’ve be de-spined!
- BT RiO not CAF-enabled
- Social Care no longer given access to the Summary Care Record
- So no one to talk to.
So CFH no longer supporting Common Assurance Process for CAF Messaging.
What the next steps will be
Given challenges, Camden and Islington Foundation trust procured Graphnet to act as a data warehouse on top of RiO Mental Health. Graphnet will then be sending Mental Health Assessment data to frameworki.
This will be via frameworki-specific web services, already delivered under the CAF programme for Online Self Assessment and Citizen Portal components, which allow third party systems to search for and create Person records and to add any (locally definable) forms to those Person records. So although functionality available to all Corelogic customers, not nationally replicable beyond the Corelogic customer group.
In addition, Corelogic is working with NHS London and both LB Camden and LB Greenwich, to integrate to the NHS London Adaptor, to be provided by Quicksilva. This will enable users of frameworki to exchange Assessment and Care Planning and Section2/5 CAF Messages (mediated via ITK Messages between frameworki and the Adaptor), with a range of other agencies, including GPs and acute hospitals. Those agencies will be able to connect to the NHS London Adaptor via a number of means, including through secure email (see NHS London presentation).
