APPLICATION FOR UPPER-DIVISION

ACCOUNTING COURSES

It is the student's responsibility to:

1.  Complete all parts of the "Application for Upper-Division Accounting Courses," sign and date, and submit it to the Accounting Department office (Bldg 164, Room 2064) in the quarter prior to enrolling in ACC 304. You may mail your completed application to:

2.  Each prerequisite core course must have been completed with a grade of "C" (2.0) or better prior to enrolling in ACC 304. The Accounting Department must be able to verify these grades or the prerequisite will be considered “not completed.” There must be proof that a transferable course has been accepted by Cal Poly as equivalent to a prerequisite course or a signed and completed “Petition to Substitute” must be on file. No exceptions will be made to these rules. Students not meeting these requirements will be dropped from ACC 304.

3.  If you are currently enrolled in a prerequisite core course, indicate "currently enrolled" in the grade column. At the end of the quarter (or as soon as possible), furnish proof of the grade earned in that class. For transfer courses must have a letter from instructor or student copy of transcript until an official grade can be viewed. Until this happens, technically the student has not met the prerequisites even though they may be allowed to enroll in ACC 304.

4.  Attach copies of unofficial transcripts and/or grade reports showing the grades earned if the core course was taken at another University. If you have successfully completed the microcomputer proficiency exam, you must provide proof.

5.  It is your responsibility to make sure you have received official transfer credit for any prerequisite core courses. Occasionally, it may be necessary to file a petition to request transfer credit prior to submitting this application. For information on these procedures, go to the Student Success Center (Room 164-1060).

Other Information:

1. The Accounting Department produces a quarterly eligibility list of students who have filed an application and met all prerequisite requirements. These eligibility lists are given to the instructor to verify the eligibility of each student who is either preregistered or tries to add the course. Students who do not turn in their application form and verification of grades to the Accounting Department prior to the first day of classes will find themselves ineligible to add the course and may be dropped, if preregistered.

2. Applications turned in after the quarter begins will be processed within 48-hours and will be given last priority for adding any upper-division accounting courses. In that case, students will receive an approval card from the Accounting Dept. to give to their instructor.
