Cal MediConnect Providers Summit

Purpose

The California Department of Health Care Servicesinvites you to an important Provider Education Summitfor providers participating inCal MediConnect. The summit will give providers, managers and other staff on the frontlines of implementing Cal MediConnect the concrete tools and information they need to fulfill the program’s promise of more integrated and coordinated care for dual eligible beneficiaries. The Summit will feature break-out sessions that bring together health plans, plan delegates (medical groups and IPAs) and other providers for practical conversations about care integration and operational issues.

1.Integrated Care Teams: Best Practices:The heart of Cal MediConnect is the enhanced care coordination that improves the health care experience for beneficiaries. This panel will share best practices in the roles of the health plans, medical groups and other providers, and discuss how providers across the spectrum can effectively communicate and collaborate on behalf of their patients.

2.Delegation 101:Plans and delegates will need to adapt practices around network management and communications as well as delegation oversight to reflect the new benefits and services delivered through Cal MediConnect. This panel will discuss how delegation should work, and share best practices on how plans and delegates can better train staff and communicate both with each other, and with consumers and providers.

  1. Engaging Consumers in Care:Many dual eligible beneficiaries are unfamiliar with managed care plans or how to navigate their new health care system. This panel will share best practices in how to communicate most effectively to beneficiaries during the on-boarding process and how to educate consumers about managed care. This panel will also share best practices for plans in locating and engaging consumers in the Health Risk Assessment process.
