The purpose of this report is to notify the Lifer Review and Recall Section (LRRS) of a request for recall. The report must make an assessment of the risk of harm posed by the offender. The report must address the following and should cross reference to any information in any previous progress reports where relevant:

1. THE CIRCUMSTANCES AND DETAILS OF EVENTS LEADING TO RECALL, and where appropriate any licence conditions which have been broken. This relates to the reasons for concern over the increase in the licensee’s risk to life and limb of others. Also confirm whether or not the licensee is subject to current supervision at the time of request.

2. THE LICENSEE’S GENERAL RESPONSE TO SUPERVISION, including details of Probation Service and/or LRRS warnings prior to this recall request; any previous breaches of conditions and any action taken; previous recalls and releases on licence. It will include general behaviour and attitude during this period of supervision.

3. A RISK ASSESSMENT – Recall action will be based on a current assessment of the risk of serious harm posed by the offender, as well as the capacity of any future licence supervision to manage them. Your assessment should include:

·  A brief analysis of the index offence, the pattern of behaviour surrounding the offence and any pattern of offending

·  An assessment of the nature of the current risk, including the potential impact and its likelihood

·  An identification of any potential victims

·  An analysis of the underlying factors that increase the likelihood of re-offending, the circumstances that make it more likely, and the immediate triggers

Endorsement and comments of ACO (or equivalent) – all recall requests must have endorsement by the Senior Manager. In particular, Out of Hours requests should be undertaken by an ACO (or equivalent grade). Please ensure that in cases where an emergency recall is requested, a named person (normally the ACO) is available for at least 30 minutes after the request is made. If there has been no contact by the Lifer Review and Recall Section during that time, the LRRS caseworker should be contacted.

[1] Where the licensee presents an immediate risk to public safety and recall action needs to be instigated by LRRS as a matter of urgency. In such cases there is not enough time to contact the Parole Board for advice prior to instigating recall action. Emergency recall will be appropriate in cases where the licensee’s behaviour presents an immediate risk of sexual or violent harm to others, regardless of the type of index offence for which he/she was originally convicted. That risk does not have to be based on the licensee being charged with any criminal offence. Delete as appropriate.

[2] Where there are concerns that a licensee represents an unacceptable risk to the public but the circumstances of the case allow LRRS the time to refer the case to the Parole Board for advice on recall action. Delete as appropriate.
