Milton Keynes End of Life Care Programme Terms and Conditions

By agreeing to participate in this Programme and signing below, you agree that your organisation will meet the following expectations detailed and will commit to full participation.

·  NMDS-SC compliance for eligibility for WDF- we must have evidence of this prior to registration that your organisation’s NMDS has been updated and is compliant for eligibility to claim for WDF. The Berkshire WDF Members Declaration Form will need to be signed

·  A recent significant personal bereavement should preclude learners from registering

·  Willing to register for QCF with City and Guilds and fill in necessary paperwork. The successful completion of this programme will result in the QCF Level 3 Award Awareness of End of Life Care plus the additional unit as explained below

·  Commitment to complete the full programme which will involve: a two hour Induction session followed by a series of workshops for each QCF unit (4 units), a further half day consolidation session, and a workbook to complete for each unit, completion of e-learning modules, and individual learning time, research and assignments will be required to be completed. Learners to remain in their cohort for the entire programme as far as possible. This award will comprise the following units:

EOL201: Understand how to work in end of life care

EOL301: Understand how to provide support when working in end of life care

EOL306: Understand how to support individuals during last days of life

(These 3 knowledge units comprise the QCF level 3 Award Awareness of End of Life Care = 10 credits)

The following unit will also be included:

EOL303: Understand advance care planning = 3 credits

·  Internet access will be required for e- learning modules and it is an expectation that the manager will confirm that participants have the necessary competency in IT skills

·  The workbooks will be available electronically if learners prefer to complete them this way. Before handing in any hard copies of your workbooks, PLEASE ask your manager to photocopy it so that you have a copy

·  All learners will be required to complete a pre and post pilot programme evaluation form

·  One learner from each organisation must be a senior care worker or manager

·  It is a condition that your organisation agrees to implement the tools to improve End of Life Care and these tools are:

Gold Standard Framework

Advance care planning

Integrated care pathway for the dying adult/End of Life plan

(All of these will be taught during the course and they will improve client care and will make a big difference to changing practice)

·  The Milton Keynes EoLC Forum has been created and meets 4 times a year and offers support in facilitating best practice and regular hot topics in EoLC are included and topics for learning. All organisations that register and complete this EoLC programme will be expected to attend at least 3 meetings a year to demonstrate their continued CPD and disseminate best practice within their organisation.

·  This programme is a step towards achieving a Milton Keynes EoLC pathway with its own quality mark

·  Should any participants from your organisation withdraw from this training programme without exceptionally good reason (to be determined by Sarah Gammon), then you agree to pay Milton Keynes Council an amount of £250 per withdrawn candidate
