BROOME COUNTY HIGH SCHOOL HOCKEY ASSOCIATION
Agreement and Release of Liability
The undersigned, individually, and by his parent or legal guardian (hereinafter collectively referred to as Releasor), acknowledge that I have voluntarily applied to participate in the Broome County High School Hockey Association for the season including all tryouts, draft sessions, practices, and games.
I am aware that the sport of ice hockey can be a dangerous and hazardous activity, and I am voluntarily participating in the activity with knowledge of the inherently dangerous nature of this sport and assume any and all risks of injury, death, or property damage.
In consideration of being permitted to participate in these activities, the undersigned, for himself/herself and his/her personal representatives, heirs and next of kin, releases, waives, discharges and covenants not to sue the Broome County High School Hockey Association, its officers, members, directors, member teams, coaches, assistant coaches, managers, and representatives (hereinafter collectively referred to as Releases) from any and all liability and for all loss or damage and any claim or damage on account of injury to the person or property or resulting in death of the Releaser, whether caused by the negligence of Releases or otherwise while the Releaser is participating in the activities of the Broome County High School Hockey Association.
I further agree for myself and my heirs, distributes, legal representatives and assigns that in the event that any claim for personal injury, death or property damage shall be prosecuted against the Releases, or any of them, I shall indemnify and save harmless the Releases from any and all such claims or causes of action by whomever made and wherever presented.
I have carefully read the Release and fully understand the contents. I am aware that this is a release of liability and a contract between myself and the Releases and sign it of my own free will. Releasor agrees that this Release, Waiver and Indemnity Agreement is intended to be as broad and inclusive as permitted by the laws of the State of New York and if any portion of the Agreement is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect.
