BPM Microsystems utilizes a Return Material Authorization (RMA) process to send hardware to the factory. Please contact BPM Microsystems Technical Support via email with this completed document to obtain a RMA number before shipping any equipment to the factory.

In most cases the result of BPWin Diagnostics are the best tool to define the problem description for specific hardware configurations that have the problem. Diagnostics can be run from Tools/Programmer Diagnostics in BPWin.

Shipping Checklist

·  Ensure the RMA number is clearly marked on the outside of the package

·  Securely pack the returning items to prevent damage in transit

·  Customer is responsible for all duties and taxes

International Shipping Documentation Instructions

For all returns outside of the United States (including Mexico and Canada):

-  Prepare a commercial invoice to be included with the shipment to include the following statement with the Item Description:

o  Equipment is being returned temporarily for repair and have no customs value, Made in the U.S., not advanced in value or improved condition. U.S. HTS#: 9801.00.1012

-  Complete CBP Form 3311 (Department of Homeland Security, U.S. Customs and Border Protection, Declaration For Free Entry Of Returned American Products)

o  CBP Form 3311 must include item #6. Reason For Return as:
“Equipment is being returned temporarily for repair and have no customs value, Made in the U.S., not advanced in value or improved condition. U.S. HTS#: 9801.00.1012”

Note: BPM Microsystems is not responsible for duties or taxes imposed as a result of improper international shipping documentation.

Evaluation and Non-Warranty RMA Purchase Order Instruction

BPM Microsystems Technical Support will request an advance Purchase Order for all non-warranty repair or evaluation RMA requests. Please click the link above to see Purchase Order Instructions.

Currently, minimum evaluation and repair charges are $250.00* per item received. RMA Purchase orders can be emailed with your completed Non-warranty RMA Questionnaire to: or faxed to 713.688.0920. Once the Purchase Order is received RMA # will be issued. At that time shipping instructions will be emailed. Once your RMA has been received and evaluated by BPM we will contact you with an estimated repair quote and request a revised purchase order for said estimate. Receipt of your revised purchase order will act as your authorization for BPM to proceed with the repairs.

Please note, calibration charges are currently $135.00* per site. (*Prices and terms are subject to change.)

Notes

-  BPM Microsystems is only responsible for items while they are in our possession

-  Customer is responsible for all freight charges for non-warranty returns.

-  Standard “bench” labor rates are currently $175.00* per hour.

-  If you chose to reject the repair estimate BPM will invoice the minimum evaluation amount of ($250.00*) per item received. Additionally, the customer will be asked if the item should be returned unrepaired or scrapped. If “scrapped” is elected the item will be disposed of and will no longer be available for return.

-  In very rare cases, the final repair charges may not match initial repair estimate. In this case BPM will contact you with the final repair estimate for your approval and revised purchase order.
