The following categories indicate the types of materials applicants must submit. Please make sure your name is on all materials. Allmaterials should be submitted as one document in Word or PDF format on a labeled CD or flashdrive. The submissions should be double spaced and properly formatted with the appropriate heading for each requirement. Any videos must be saved as .mp4 files and must be smaller than 250 MB. Use this form as a checklist to ensure that all elements required to be submitted are included in your application. Return this form and your materials to the Bone Scholarship Selection Committee, c/o Honors Program, no later than March 2, 2016 (4:00 PM).

LETTERS OF NOMINATION AND RECOMMENDATION

PurposeTo provide the selection committee with insight about the nominee's character, work ethic and accomplishments, from people who know him/her personally and/or professionally.

CriteriaYou must submit a total of five letters. Two are letters of nomination from your department (already submitted), and three additional letters of recommendation may come from personal and/or professional contacts (such as work supervisors and ISU personnel). Letters can be submitted in paper format to Bone Scholarship Selection Committee, c/o Illinois State University, 6100 Honors Program, Normal, IL 61790-6100,electronically to , or as part of your application portfolio.

The letters should include:

•the relationship between the writer and the nominee.

•a description of the nominee's character, work ethic, and accomplishments.

•a discussion of how the nominee qualifies as a Bone Scholar.

SIGNIFICANT COURSES

PurposeTo provide the selection committee with insight about what intrigues, excites or inspires the nominee in his/her role as a student.

CriteriaYou must identify no more than four courses taken at Illinois State and discuss why these courses have had a significant impact on your personal and/or professional life. Also indicate if the course was a required course for your major or if volunteer work was done as part of a course.

ACTIVITIES AND ORGANIZATIONS

PurposeTo provide the selection committee with insight regarding the breadth and depth of the nominee's academic, social, work and/or civic involvement within and outside the university.

CriteriaYou must submit a listing and reflections of your participation in activities and organizations associated with Illinois State and those associated with the local and/or home community. Please do not submit high school activities and organizations. Activities and organizations should be in resume format (chronological order and bulleted).

The descriptions should include:

•dates and length of participation.

•duties and/or responsibilities.

•positions and/or offices held.

•honors and/or awards received.

PROJECT

PurposeTo provide the selection committee with insight about the nominee's abilities as a scholar/student

CriteriaYou must submit one example of a project that represents your "best work." Papers, literary works, art projects, portfolios, and journals are examples of suitable projects (at your request, the materials will be returned to you after May, 2016). If you are submitting a group project, be sure to describe and/or indicate your contribution.

PROJECT CRITIQUE

PurposeTo provide the selection committee with insight about the nominee's ability to analyze critically his/her own work.

CriteriaYou must submit a critique of your project that discusses its significance and how the project could have been improved.

ESSAY

PurposeTo provide the selection committee with insight about the nominee's ability to think critically and/or creatively and express him/herself in writing (including style, organization and grammar).

CriteriaYou must compose an essay, of not more than 600 words, that addresses the following questions:

•What are the major challenges in your field of study now and in the immediate future?

•In what new directions will the field have to go in order to respond to those challenges?

•How will these changes in the field affect you personally?

•Has your curriculum prepared you to deal with these changes?
