BLACK FOREST SADDLE CLUB
NOTICE, CONSENT, ASSUMPTION OF RISK, INDEMNIFICATION, AND HOLD HARMLESS AGREEMENT
As a member (or guest) of the Black Forest Saddle Club, you or your child are eligible to use certain facilities and participate in various equestrian activities. Do not participate and do not allow your child or your animals to participate by use of the Club's facilities and/or activities unless you agree with all of the following statements:
1. Equestrian activities of all kinds are dangerous.
2. You are aware and know these dangers exist.
3. You voluntarily give your unqualified permission, in spite of the inherent dangers,
for you, your child, and your animals to participate in all aspects of the Club's
programs, including, but not limited to: travel and use of any facilities for
recreation, practice, or contests.
4. That you will INDEMNIFY and HOLD HARMLESS the Black Forest Saddle Club,
its officers, directors, agents or employees, and the owners and operators of any
facilities used, or injury sustained by you, your child, or any animal, as a participant
in the program.
5. All equipment shall be the responsibility of the parents and participants and shall
not be the responsibility of any other person or entity.
6. The usual and probable consequences of participating in the equestrian program
are physical contact with resulting personal injury of varying degree and severity.
7. You agree that neither those in charge nor the sponsors, shall be held responsible
in case of damage or injury, whether the injury is sustained while an activity or
the facilities are supervised at the time of damage or injury.
PARTICIPANTS ARE ESPECIALLY URGED TO BE CAREFUL, CAUTION IS THE BEST POSSIBLE SAFEGUARD.
I HAVE READ THE FOREGOING, UNDERSTAND IT, GIVE MY CONSENT TO IT AND CONSENT TO MY CHILD AND ANIMALS PARTICIPATING, ASSUMING FULL AND COMPLETE RISK AND RESPONSIBILITY, AND AGREEING TO THE "HOLD HARMLESS" AND "INDEMNIFICATION" AGREEMENT ABOVE SET FORTH.

IN ORDER TO PARTICIPATE IN CLUB SPONSORED EVENTS, YOU MUST SIGN THIS RELEASE (IF UNDER 18 PRINT NAME AND HAVE GUARDIAN SIGN).

UNDER COLORADO LAW, AN EQUINE PROFESSIONAL IS NOT LIABLE FOR ANY INJURY TO OR THE DEATH OR A PARTICIPANT IN EQUINE ACTIVITIES RESULTING FROM THE INHERENT RISKS OF EQUINE ACTIVITIES PURSUANT TO SECTION 13-21-119, COLORADO REVISED STATUTES.
