Marjorie Nicholas, PhD CCC-SLP, is the Associate Chair of the Department of Communication Sciences and Disorders at the MGH Institute of Health Professions in Boston, MA.Prior to joining the faculty at MGH IHP, she worked for over 15 years as a speech-language pathologist and research associate at the Boston VA Medical Center.She has co-authored numerous research articles on aphasia, language in normal aging, and dementia. She is co-author of various assessment and treatment materials for aphasia including the Boston Assessment of Severe Aphasia (BASA), the Sentence Production Program for Aphasia (SPPA), the Narrative Story Cards, and the C-Speak Aphasia software program for alternative communication.At the MGH Institute of Health Professions she founded the Aphasia Center, operated as a clinical training site for graduate students that is open to individuals from the community who have aphasia and related neurogenic communication disorders. Her research interests focus on the treatment of severe aphasia, and in particular on the cognitive and neuroanatomical mechanisms underlying successful response to communication treatment.

Lisa Connor, PhD Dr. Connor earned her Bachelor’s degree in Psychology from Johns Hopkins University and her Master’s and PhD degrees in Experimental Psychology from Washington University. After graduating from the PhD program, she completed post-doctoral training in cognitive aging at Georgia Institute of Technology, then in adult communicative disorders at the Aphasia Research Center at the Boston Veteran’s Administration Hospital. While in Boston, Dr. Connor taught at Brandeis University. During her time at the Aphasia Research Center, Dr. Connor was Assistant Director of the Language and the Aging Brain laboratory and Associate Director of the Center. In 2001, Dr. Connor returned to Washington University to Dr. Maurizio Corbetta’s laboratory to learn functional neuroimaging techniques. She joined the Program in Occupational Therapy faculty in 2006. Dr. Connor is Director of the Cognitive Rehabilitation Research Group Stroke Registry.

Overview

Dr.Nicholas and Dr. Connor will present the latest evidence-based information on the assessment and intervention of aphasia, using the A-FROM model (Living with Aphasia, Framework for Outcome Measurement; Kagan, Simmons-Mackie, Rowland, Huijbregts, Shumway, McEwen, Threats, & Sharp, 2008), which provides a guide for measuring outcomes beyond those typically measured in impairment-focused treatment models. A-FROM describes four domains with the person living with aphasia (PWA) at the intersecting hub of all domains. This presentation will discuss the assessment and intervention of PWA within each of these domains. The workshop is divided into 5 sections, one for each of the domains and a final integration section. Current research in assessment and treatment of PWA will be discussed and hands-on activities will be conducted to help clinicians understand how their clinical effectiveness could be enhanced by application of the A-FROM model. The overall goal of the session is to encourage a broadening of the scope of intervention beyond the impairment domain which typically has been the primary, if not the only, focus of intervention.

Learner Objectives

Participants will be able to:

  1. Articulate how the A-FROM model can guide treatment goals for people with aphasia (PWA).
  2. Describe assessment tools that are appropriate for evaluation of PWA within the A-FROM domains.
  3. Design actual treatment plans and suggest specific goals that address each of the four domains of the A-FROM model using a case-based learning format.
  4. Discuss the benefits of an integrated treatment plan for PWA that addresses outcomes beyond impairment.

Workshop participant group learning activity: Using results from nonverbal cognition assessment to guide treatment planning of communication impairment.

10:45 -11:00 Short break

11:00- 12:30 The Participation Domain: Assessment- Card Sort, LIV Cards research studies, Reintegration to Normal Living, Stroke Impact Scale Participation Domain. Treatment- Community groups, book clubs, etc.Workshop participant group learning activity: Design goals and activities combining data obtained from assessments from both the impairment and the participation domain.

12:30-1:15 LUNCH

1:15 – 2:15 The Environment Domain: Assessment- The MOSE and other assessments of the environment

Treatment- Training families in Supported Communication; Other environmental manipulations

Workshop participant group learning activity: Design goals and activities that address 3 domains (impairment, environment, and participation)

2:15- 3:15 The Personal Domain: Assessment- Assessment of personal characteristics relevant to PWA; visual analogue

mood and behavior scales.Treatment- Counseling, other treatments addressing personal characteristics

Workshop participant group learning activity:Design goals and activities that

address the personal domain along with at least one other domain.

3:15- 3:30 Short break

3:30-4:45 Integration and Summary. Debriefing with groups about the learning activities. Workshop participant group learning activities:As a group write a summary of the advantages of the A-FROM approach.First steps in designing outcomes research for all domains.
