Recognition is the critical element in the Process Excellence Awards. If your entry is selected for First Place, Process Excellence Awards2016will provide a complimentary engraved trophy signifying youraccomplishment and up to five certificates for the team involved in helping create a winningProcess Excellence Program.

QUALIFICATION QUESTIONS
The percentage value is denoted and all categories together equal 100% of the application score. Each question is to be limited to 1 page per response; additional information may be provided in appendices but may not be reviewed.
It is important to provide data to support your answers, to give the judges a good understanding of the impact it has had on your Process Excellence Program.
The total length of the submission should not be more than 8 pagesincluding the cover page (Arial font size 10). Please include at least 1 photo, but no more than 10 photographs showing your staff and centre in an appendix.

1) Describe the desired future culture and the driving forces behind the change initiative. Please provide supporting data.
